独活寄生汤联合双氯芬酸钠缓释片应用于老年寒湿痹阻型腰椎间盘突出症患者的疗效

Effect of Duhuo Jisheng Decoction Combined with Diclofenac Sodium Sustained Release Tablets on Elderly Patients with Lumbar Disc Herniation of Cold Dampness Obstruction Type

目的探讨独活寄生汤联合双氯芬酸钠缓释片治疗老年寒湿痹阻型腰椎间盘突出症的效果.方法选取汉中市勉县骨伤科医院2021年3月—2023年3月收治的90例老年寒湿痹阻型腰椎间盘突出症患者为研究对象,将其随机分为对照组及观察组,每组45例.对照组采用双氯芬酸钠缓释片治疗,观察组在此基础上采用独活寄生汤治疗.对比两组患者的临床疗效﹑中医证候积分﹑炎性因子水平及腰椎功能.结果治疗后,观察组的腰膝酸软﹑畏寒肢冷及腰酸痛﹑下肢沉重等中医证候积分均低于对照组,组间差异有统计学意义(P<0.05).观察组的治疗总有效率为97.78%,高于对照组的82.22%,差异有统计学意义(P<0.05).治疗后,观察组的白细胞介素-6﹑肿瘤坏死因子-α水平均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的各项日本骨科协会评估治疗评分均高于对照组,组间差异有统计学意义(P<0.05).结论老年寒湿痹阻型腰椎间盘突出症患者采用独活寄生汤联合双氯芬酸钠缓释片治疗,可提高临床疗效,改善机体炎性因子表达水平,缓解疾病症状,提高腰椎功能,值得临床推广使用.

Objective To explore the effects of Duhuo Jisheng Decoction combined with diclofenac sodium sustained release tablets on the efficacy and inflammatory factor expression in elderly patients with lumbar disc herniation of cold dampness obstruction type.Methods 90 elderly patients with lumbar disc herniation of cold dampness obstruction type admitted to Mian County Orthopedic Hospital in Hanzhong City from March 2021 to March 2023 were selected as the study objects and were randomly divided into a control group and an observation group,with 45 cases in each group.The control group was treated with diclofenac sodium sustained release tablets,and the observation group was treated with Duhuo Jisheng Decoction on this basis.The clinical efficacy,TCM syndrome scores,inflammatory factor levels and lumbar function were compared between the two groups.Results After treatment,the traditional Chinese medicine syndrome scores such as lumbago and knee weakness,cold limbs,lumbago and lower limb heaviness in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of the observation group was 97.78%,higher than 82.22%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-6 and tumor necrosis factor-α in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the Japanese Orthopaedic Association treatment evaluated scores in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Duhuo Jisheng Decoction combined with diclofenac sodium sustained release tablets can improve the clinical efficacy in elderly patients with lumbar disc herniation of cold dampness obstruction type,improve the expression level of inflammatory factors,alleviate the disease symptoms,and improve the function of lumbar vertebra,which is worthy of clinical promotion.
