负压创面治疗技术辅助削痂植皮术治疗成年深度烧伤对患者疼痛程度、创面愈合情况的影响

The Effect of Negative Pressure Wound Treatment Technology Assisted Scab Removal and Skin Grafting on the Pain Level and Wound Healing of Adult Deep Burns Patients

目的探究负压创面治疗技术联合削痂植皮术对成年深度烧伤患者的影响。方法选取该院2022年1月—2023年6月收治的70例成年深度烧伤患者为研究对象,按随机数字表法将其分为对照组与观察组,各35例。对照组行削痂植皮术,观察组在对照组基础上加以负压创面治疗技术。比较两组患者的创面愈合情况、疼痛程度、炎症因子水平及并发症发生情况。结果观察组肉芽生长时间为(8.70±1.59)d、创面愈合时间为(18.63±2.18)d,均短于对照组的(12.41±2.36)d、(24.59±2.67)d,换药频次为(3.57±0.83)次,少于对照组的(6.36±1.59)次,并发症发生率为5.71%,低于对照组的22.86%,组间差异有统计学意义(P<0.05)。治疗前,两组疼痛视觉模拟(VAS)评分与白细胞计数(WBC)、C反应蛋白(CRP)、白细胞介素-6(IL-6)相比,组间差异无统计学意义(P>0.05);治疗后,观察组VAS评分为(3.72±0.54)分、WBC为(4.20±1.33)×109/L、CRP为(3.48±0.95)mg/L、IL-6为(110.54±8.25)ng/L,均低于对照组的(4.67±0.63)分、(7.36±1.58)×109/L、(7.21±1.22)mg/L、(129.84±10.53)ng/L,组间差异有统计学意义(P<0.05)。结论负压创面治疗辅助削痂植皮术有助于成年深度烧伤患者创面迅速愈合,减轻疼痛,降低炎症因子水平,且并发症少。

Objective To explore the impact of negative pressure wound treatment technology combined with scab removal and skin grafting on adult deep burn patients.Methods 70 adult patients with deep burn admitted to the hospital from January 2022 to June 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 35 cases in each group.The control group underwent scab grafting,and the observation group was treated with negative pressure wound on the basis of the control group.Wound healing,pain,inflammatory factors and complications were compared between the two groups.Results The granulation growth time and wound healing time of the observation group were(8.70±1.59)d and(18.63±2.18)d,both of which were shorter than(12.41±2.36)d and(24.59±2.67)d of the control group,and the dressing change frequency was(3.57±0.83)times,less than(6.36±1.59)times of the control group,the incidence of complications was 5.71%,lower than 22.86%in the control group,and the differences between groups were statistically significant(P<0.05).Before treatment,the pain Visual Analogue Scale(VAS)score,white blood cell count(WBC),C-reactive protein(CRP)and interleukin-6(IL-6)were not significantly different between the two groups(P>0.05);after treatment,VAS score was(3.72±0.54)points,WBC was(4.20±1.33)×109/L,CRP was(3.48±0.95)mg/L,IL-6 was(110.54±8.25)ng/L,they were lower than(4.67±0.63)points,(7.36±1.58)×109/L,(7.21±1.22)mg/L,(129.84±10.53)ng/L of control group,and the differences between groups were statistically significant(P<0.05).Conclusion Negative pressure wound treatment assisted with scab removal and skin grafting can help adult deep burn patients quickly heal their wounds,reduce pain,lower levels of inflammatory factors,improve quality of life,and have fewer complications.
