医护一体化护理模式联合ERAS干预在胸外科手术患者围术期中的应用

Application of Integrated Nursing Model Combined with ERAS Intervention in Perioperative Period of Thoracic Surgery Patients

目的研究医护一体化护理模式联合快速康复外科(ERAS)在胸外科手术患者围术期中的应用效果。方法选取2020年10月—2022年10月我院收治的82例胸外科手术患者为研究对象,采用随机信封法将其分为对照组与研究组,各41例。两组均接受手术治疗,对照组采用传统围术期护理干预,研究组在此基础上采用医护一体化护理模式联合ERAS干预。比较两组术后恢复情况、术后并发症发生率、不同时间段疼痛视觉模拟(VAS)评分及Kolcaba简化舒适状况量表(GCQ)评分。结果研究组术后经口进食时间、肛门排气时间、下床活动时间、拔管时间及住院时间均短于对照组,组间差异有统计学意义(P<0.05);研究组术后并发症发生率4.88%低于对照组的19.51%,差异有统计学意义(P<0.05);术后12、24、48、72 h,研究组VAS、GCQ评分均低于对照组,组间差异有统计学意义(P<0.05)。结论医护一体化护理模式联合ERAS干预能降低胸外科手术患者并发症发生风险,可有效减轻患者疼痛感,提高舒适度,有利于加速术后康复。

Objective To study the effect of integrated nursing model combined with rapid rehabilitation surgery(ERAS)in patients undergoing thoracic surgery during perioperative period.Methods 82 thoracic surgery patients admitted to our hospital from October 2020 to October 2022 were selected as the study objects,and were divided into a control group and a study group by random envelope method,with 41 cases in each group.Both groups received surgical treatment,the control group received traditional perioperative nursing intervention,and the study group received integrated nursing mode combined with ERAS intervention on this basis.The postoperative recovery,postoperative complication rate,pain Visual Analogue Scale(VAS)and Kolcaba Simplified Comfort Scale(GCQ)scores were compared between the two groups.Results The oral feeding time,anal exhaust time,getting out of bed time,extubation time and hospitalization time of the study group were shorter than those of the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the study group was 4.88%lower than 19.51%in the control group,and the difference was statistically significant(P<0.05).At 12,24,48 and 72 h after operation,VAS and GCQ scores of the study group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Combined with ERAS intervention can reduce the risk of complications in thoracic surgery patients,effectively reduce pain,improve comfort,and accelerate postoperative rehabilitation.
