肺康复训练配合BiPAP在老年慢性阻塞性肺疾病伴慢性呼吸衰竭患者中的应用效果

摘要 目的探讨肺康复训练配合双水平气道正压通气(BiPAP)在老年慢性阻塞性肺疾病(COPD)伴慢性呼吸衰竭患者中的应用效果。方法选择2022年4月—2023年2月江苏省省级机关医院收治的100例老年COPD伴慢性呼吸衰竭患者为研究对象,采用随机数字表法将其分为对照组及观察组,每组50例。对照组采用BiPAP通气治疗,观察组采用肺康复训练配合BiPAP治疗,均持续治疗8周。对比两组患者的肺功能、日常生活活动能力及运动耐力。结果治疗后,观察组的第1秒用力呼气容积、用力肺活量、呼气峰值流速、1秒率分别为(2.02±0.30)L、(2.97±0.42)L、(3.97±0.23)L/s、(72.17±5.64)%,均大于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的Barthel指数评分为(81.52±2.34)分,高于对照组,6 min步行距离为(347.66±22.43)m,长于对照组,组间差异有统计学意义(P<0.05)。结论肺康复训练配合BiPAP在老年COPD伴慢性呼吸衰竭患者中的应用效果显著,可改善其肺功能,提高其日常生活能力及运动耐力。 Objective To explore the effect of lung rehabilitation training combined with Bi-level positive airway pressure(BiPAP)in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD)and chronic respiratory failure.Methods 100 elderly patients with COPD and chronic respiratory failure admitted to Jiangsu Provincial Government Hospital from April 2022 to February 2023 were selected as the study objects and were divided into a control group and an observation group by random number table method,with 50 cases in each group.The control group was treated with BiPAP,and the observation group was treated with lung rehabilitation training combined with BiPAP.Both groups were treated for 8 weeks.The lung function,activities of daily living and exercise endurance were compared between the two groups.Results After treatment,the forced expiratory volume in one second,forced vital capacity,peak expiratory flow rate and forced expiratory volume in one second/forced vital capacity in the observation group were(2.02±0.30)L,(2.97±0.42)L,(3.97±0.23)L/s and(72.17±5.64)%,respectively,which were greater than those in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the Barthel index score in the observation group was(81.52±2.34)points,which was higher than that in the control group,and the 6-min walking distance was(347.66±22.43)m,which was longer than that in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Lung rehabilitation training combined with BiPAP can improve lung function,daily living ability and exercise endurance in elderly patients with COPD and chronic respiratory failure.
