持续被动运动锻炼配合髋外展肌强化训练对老年全膝关节置换术患者术后平衡功能、膝关节功能的影响

Effect of Continuous Passive Exercise Combined with Hip Abductor Strengthening Training on Postoperative Balance Function and Knee Function in Elderly Patients with Total Knee Arthroplasty

目的探讨持续被动运动锻炼配合髋外展肌强化训练对老年全膝关节置换术(TKA)患者术后平衡功能、膝关节功能的影响。方法选取2020年1月—2023年1月本院收治的行TKA的100例老年患者为研究对象,按随机数字表法将其分为对照组和观察组,各50例。对照组采取髋外展肌强化训练,观察组在对照组基础上加用持续被动运动锻炼。比较两组平衡功能评分、膝关节功能评分、膝关节活动度、日常生活能力评分及并发症发生情况。结果干预前,两组Berg平衡量表(BBS)评分、膝关节功能量表(HSS)评分、膝关节活动度及Barthel指数(BI)评分比较,组间差异无统计学意义(P>0.05);干预后,观察组BBS评分(48.23±4.13)分、HSS评分(85.53±4.35)分、膝关节活动度(107.73±9.64)°、BI评分(87.66±6.31)分均高于对照组的(42.89±4.07)分、(76.84±4.19)分、(90.35±8.35)°、(72.18±6.26)分,组间差异有统计学意义(P<0.05)。观察组并发症发生率6.00%低于对照组的20.00%,差异有统计学意义(P<0.05)。结论行TKA的老年患者术后采用持续被动运动锻炼结合髋外展肌强化训练效果显著,值得推广。

Objective To investigate the effect of continuous passive exercise combined with intensive hip abduction training on postoperative balance and knee function in elderly patients with total knee arthroplasty(TKA).Methods 100 elderly patients who underwent TKA admitted to our hospital from January 2020 to January 2023 were selected as the research subjects.They were divided into a control group and an observation group using a random number table method,with 50 patients in each group.The control group adopted the intensive training of the hip abductor muscle,and the observation group added the continuous passive exercise on this basis.Compare the balance function score,knee joint function score,knee joint range of motion,daily living ability score,and incidence of complications between the two groups.Results Before intervention,there was no statistically significant difference between the two groups in the Berg Balance Scale(BBS)score,Knee Function Scale(HSS)score,knee range of motion,and Barthel Index(BI)score(P>0.05).After intervention,the BBS score(48.23±4.13)points,HSS score(85.53±4.35)points,knee range of motion(107.73±9.64)°,and BI score(87.66±6.31)points in the observation group were higher than(42.89±4.07)points,(76.84±4.19)points,(90.35±8.35)°,and(72.18±6.26)points in the control group,with statistically significant differences between the groups(P<0.05).The incidence of complications in the observation group was 6.00%lower than 20.00%in the control group,with a statistically significant difference(P<0.05).Conclusion Elderly patients with TKA used continuous passive exercise combined with hip abductor training,which has remarkable results and is worth popularizing.
