氢氧化钙糊剂填充窝洞辅助开髓减压引流术治疗急性牙髓炎患儿的临床效果观察

Clinical Observation on the Treatment of Acute Pulpitis in Children with Calcium Hydroxide Paste Filling Cavity and Assisted Pulp Opening Decompression and Drainage

目的探讨氢氧化钙糊剂填充窝洞辅助开髓减压引流术治疗在急性牙髓炎患儿中的应用效果.方法选取2021年5月—2022年6月兰陵县人民医院收治的90例急性牙髓炎患儿为研究对象,随机将其分为对照组和观察组.两组均进行开髓减压引流术治疗,对照组(n=45)采用氧化锌丁香油糊剂填充窝洞辅助,观察组(n=45)采用氢氧化钙糊剂填充窝洞辅助,对比两组的疼痛程度、炎症指标、牙周健康.结果观察组治疗后1、3、7 d的疼痛视觉模拟评分分别为(3.92±0.48)分、(2.19±0.41)分、(1.45±0.26)分均低于对照组的(4.20±0.56)分、(2.58±0.49)分、(1.80±0.34)分,组间差异有统计学意义(P<0.05).治疗后7d,观察组血清白细胞介素-6、白细胞介素-8、白三烯B4、Toll样受体4水平均低于对照组,观察组菌斑指数、牙龈指数、龈沟出血指数评分均低于对照组,龈沟探诊深度低于对照组,组间差异有统计学意义(P<0.05).结论氢氧化钙糊剂填充窝洞辅助开髓减压引流术治疗对急性牙髓炎患儿疼痛程度、炎症指标均有减轻和降低的作用,且利于改善患儿的牙周健康.

Objective To investigate the effect of calcium hydroxide paste filling cavity assisted open pulp decompression and drainage in children with acute pulpitis.Methods 90 children with acute pulpitis in Lanling County People's Hospital from May 2021 to June 2022 were selected as the research objects and randomly divided into a control group and an observation group.The control group(n=45)was treated with zinc oxide clove oil paste to fill the cavity,and the observation group(n=45)was treated with calcium hydroxide paste to fill the cavity.The degree of pain,inflammatory indicators,and periodontal health were compared between the two groups.Results The pain Visual Analogue Scale scores of the observation group were(3.92±0.48)points,(2.19±0.41)points,(1.45±0.26)points,respectively,which were lower than(4.20±0.56)points,(2.58±0.49)points,(1.80±0.34)points of the control group.The difference between the two groups was statistically significant(P<0.05).7 d after treatment,the levels of serum interleukin-6,interleukin-8,leukotriene B4,Toll-like receptor 4 in the observation group were lower than those in the control group,the plaque index,gingival index,sulcus bleeding index scores in the observation group were lower than those in the control group,and the gingival sulcus probing depth was lower than that in the control group,the differences between the two groups were statistically significant(P<0.05).Conclusion The treatment of cavity filling with Calcium hydroxide paste assisted pulpotomy decompression and drainage can reduce the pain and inflammatory indexes of children with acute pulpitis,and is beneficial to improving the periodontal health of children.
