本体感觉训练对膝骨性关节炎全膝关节置换术后患者膝关节功能、下肢肌力及平衡能力的影响

Effect of Proprioceptive Training on Knee Function,Lower Limb Muscle Strength and Balance in Knee Osteoarthritis Patients after Total Knee Arthroplasty

摘要 目的分析膝骨性关节炎全膝关节置换术后患者进行本体感觉训练对其膝关节功能、下肢肌力及平衡能力的影响.方法选取2020年9月—2021年12月于复旦大学附属华东医院行膝骨性关节炎全膝关节置换术的患者90例为研究对象,采用随机数字表法将其分为对照组及研究组,每组45例.术后,对照组进行常规康复训练,研究组在此基础上进行本体感觉训练.对比两组患者的膝关节功能、下肢肌力及平衡能力.结果干预后,研究组的西安大略和麦克马斯特大学骨关节炎指数中关节功能、关节疼痛、关节僵硬评分均低于对照组,组间差异有统计学意义(P<0.05);研究组的屈肌及伸肌总功、单次最大做功量、峰力矩均高于对照组,组间差异有统计学意义(P<0.05);研究组的Berg平衡量表评分为(47.28±4.70)分,高于对照组,动态平衡功能起立-行走测试时间为(20.09±4.65)s,短于对照组,组间差异有统计学意义(P<0.05).结论本体感觉训练联合常规康复训练能有效改善膝骨性关节炎全膝关节置换术后患者的膝关节功能,提高其下肢肌力及平衡能力,值得临床推广使用. Objective To analyze the effects of proprioceptive training on knee function,lower limb muscle strength and balance in knee osteoarthritis patients after total knee arthroplasty.Methods 90 knee osteoarthritis patients who underwent total knee arthroplasty in Huadong Hospital Affiliated to Fudan University from September 2020 to December 2021 were selected as the study objects,and were divided into a control group and an research group by random number table method,with 45 cases in each group.After operation,the control group received routine rehabilitation training,and the research group received proprioceptive training on this basis.The knee joint function,lower limb muscle strength and balance ability were compared between the two groups.Results After intervention,the joint function,joint pain and joint stiffness scores of the Western Ontario and McMaster Universities osteoarthritis index in the study group were lower than those in the control group,the differences between the groups were statistically significant(P<0.05);the total work of flexor and extensor muscles,the maximum single work amount and peak moment of the study group were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05);the Berg balance scale score of the study group was(47.28±4.70)points,higher than that of the control group,and the timed up and go test time of dynamic balance function was(20.09±4.65)s,shorter than that of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Proprioceptive training combined with routine rehabilitation training can effectively improve the knee function of patients with knee osteoarthritis after total knee arthroplasty,and improve their lower limb muscle strength and balance ability,which is worthy of clinical application.
