托吡酯联合拉莫三嗪对癫痫患者血清NSE、GFAP、S100B水平及认知功能的影响

Effects of Topiramate Combined with Lamotrigine on Serum NSE,GFAP,S100B Levels and Cognitive Function in Epilepsy Patients

目的探究托吡酯联合拉莫三嗪对癫痫患者血清神经元特异性烯醇化醇(NSE)、胶质酸性纤维蛋白(GFAP)、S100B水平及认知功能的影响。方法选择2020年11月—2022年5月济南市济阳区中医医院收治的110例癫痫患者为研究对象,按随机数字表法将其分为对照组及观察组,每组55例。对照组采用拉莫三嗪治疗,观察组采用托吡酯联合拉莫三嗪治疗,均持续6个月。比较两组患者的临床疗效、血清水平、认知功能、生活质量及不良反应发生情况。结果观察组的治疗总有效率为94.54%,高于对照组的81.82%,差异有统计学意义(P<0.05)。治疗后,观察组的NSE、GFAP、S100B蛋白水平均低于对照组,组间差异有统计学意义(P<0.05);观察组的简易精神状态量表评分、蒙特利尔认知评估量表评分、生活质量综合评定量表各维度评分均高于对照组,组间差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论采用托吡酯联合拉莫三嗪治疗癫痫的效果理想,可显著改善患者的血清指标及认知功能,提高其生活质量,且安全性较高。

Objective To investigate the effects of topiramate combined with lamotrigine on serum NSE,GFAP,S100B levels and cognitive function in patients with epilepsy.Methods 110 patients with epilepsy admitted to Jiyang District Hospital of Traditional Chinese Medicine in Jinan City from November 2020 to May 2022 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 55 cases in each group.The control group was treated with lamotrigine,and the observation group was treated with topiramate combined with lamotrigine.Both groups were treated for 6 months.The clinical efficacy,serum index levels,cognitive function,quality of life and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.54%,higher than 81.82%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of neuron-specific enolase,glial fibrillary acidic protein and S100B protein in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the scores of the mini-mental state examination,the Montreal cognitive assessment and all dimensions of the generic quality of life inventory-74 in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Topiramate combined with lamotrigine is effective in the treatment of epilepsy,which can significantly improve the serum indexes and cognitive functions of patients,improve their quality of life,and have high safety.
