全肩关节镜手术对肩袖损伤患者关节功能及疼痛视觉模拟评分的影响

Effect of Total Arthroscopy on Joint Function and VAS Score in Patients with Rotator Cuff Injury

目的探究全肩关节镜与关节镜辅助下切开治疗对肩袖损伤患者关节功能及疼痛视觉模拟(VAS)评分的影响。方法选择2022年1月—2022年10月桂林市中医医院收治的160例肩袖损伤患者为研究对象。按随机数字表法将研究对象分为对照组和观察组,对照组(n=80)采用关节镜下小切口手术治疗,观察组(n=80)实施全肩关节镜手术治疗,两组均随访6个月。结果治疗后,观察组肩关节评分系统评分为(30.52±4.02)分,美国肩肘外科协会评分为(89.84±5.61)分,均高于对照组的(25.13±3.84)分、(82.58±4.78)分;VAS评分为(2.21±0.63)分,低于对照组的(4.15±1.02)分;外旋角度为(133.82±15.44)°,内旋角度为(56.39±6.37)°,前屈角度为(132.72±19.21)°,后伸角度为(40.14±3.16)°,均大于对照组的(110.54±13.49)°、(46.87±5.15)°、(112.58±18.48)°、(31.82±2.83)°;生活质量评分为(86.85±4.31)分,高于对照组(74.57±4.13)分,组间差异有统计学意义(P<0.05)。结论与关节镜辅助下切开术相比,全肩关节镜在肩袖损伤患者中效果更佳,能够有效减轻患者疼痛,扩大肩关节活动度,促进肩关节功能与生活质量改善。

Objective To explore the effects of total shoulder arthroscopy and arthroscopic incision on joint function and pain Visual Analogue Scale(VAS)score of patients with rotator cuff injury.Methods 160 patients with rotator cuff injury admitted to Guilin Hospital of Traditional Chinese Medicine from January 2022 to October 2022 were selected as the research objects and were divided into a control grup and an observation group according to random number table method.The control group(n=80)was treated with arthroscopic small incision surgery,and the observation group(n=80)was treated with total shoulder arthroscopic surgery.Both groups were followed up for 6 months.Results After treatment,the shoulder joint scoring system score was(30.52±4.02)points,and American Shoulder and Elbow Surgery Association score was(89.84±5.61)points in the observation group,which were higher than(25.13±3.84)points,(82.58±4.78)points of the control group;the VAS score of the observation group was(2.21±0.63)points,lower than(4.15±1.02)points of the control group;the external rotation angle was(133.82±15.44)°,the internal rotation angle was(56.39±6.37)°,the forward flexion angle was(132.72±19.21)°,and the back extension angle was(40.14±3.16)°,which were greater than(110.54±13.49)°,(46.87±5.15)°,(112.58±18.48)°and(31.82±2.83)°of the control group;the quality of life score of the observation group was(86.85±4.31)points,which was higher than(74.57±4.13)points of the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion Compared with Arthroscopy assisted incision,total shoulder Arthroscopy is more effective in patients with rotator cuff injury,which can effectively relieve pain,expand the range of motion of Shoulder joint,and promote the improvement of Shoulder joint function and quality of life.
