高强度间歇性心脏康复训练联合体外反搏对冠心病患者心肺适能及心功能的影响

摘要 目的探究高强度间歇性心脏康复训练联合体外反搏对冠心病患者心肺适能及心功能的影响。方法选择2020年7月—2022年8月平原县第一人民医院收治的102例冠心病患者为研究对象,按随机数字表法分为对照组和观察组,各51例。对照组进行常规运动训练,观察组于对照组基础上实施高强度间歇性心脏康复训练联合体外反搏,均持续干预8周。比较两组心肺适能、心功能、生活质量。结果干预前,两组心肺适能、心功能、生活质量比较,组间差异无统计学意义(P>0.05);干预后,观察组峰值公斤摄氧量(27.05±2.84)mL/(kg•min)、无氧阈值(17.45±1.49)mL/(kg•min)、峰值氧脉搏(12.69±1.65)mL/beat均高于对照组,左室舒张末期内径(43.76±2.55)mm、左室收缩末期内径(23.60±2.16)mm均短于对照组,左室射血分数(52.35±6.85)%高于对照组,生活质量综合评定问卷中躯体功能评分(78.37±5.75)分、心理功能评分(79.12±5.67)分、社会功能评分(81.96±5.58)分、物质生活状态评分(81.23±5.16)分均高于对照组,组间差异有统计学意义(P<0.05)。结论对冠心病患者实施高强度间歇性心脏康复训练与体外反搏可改善心肺适能、心功能,改善生活质量。 Objective To explore the effect of high intensity intermittent cardiac rehabilitation training combined with external counterpulsation on cardiopulmonary fitness and cardiac function in patients with coronary heart disease.Methods 102 patients with coronary heart disease admitted to Pingyuan County First People's Hospital from July 2020 to August 2022 were selected as the study objects and were randomly divided into a control group and an observation group,51 patients in each group.The control group received routine exercise training,and on this basis,the observation group received high intensity intermittent cardiac rehabilitation training combined with external counterpulsation,with continuous intervention for 8 weeks.Cardiopulmonary fitness,cardiac function and quality of life were compared between the two groups.Results Before the intervention,there was no statistically significant difference in cardiopulmonary fitness,cardiac function and quality of life between the two groups(P>0.05);after intervention,the peak kilogram oxygen uptake(27.05±2.84)mL/(kg•min),anaerobic threshold(17.45±1.49)mL/(kg•min),peak oxygen pulse(12.69±1.65)mL/beat in the observation group were higher than those in the control group,the left ventricular end-diastolic diameter(43.76±2.55)mm,left ventricular end-systolic diameter(23.60±2.16)mm were shorter than those in the control group,and the left ventricular ejection fraction(52.35±6.85)%was higher than that in the control group,the scores of physical function(78.37±5.75)points,psychological function(79.12±5.67)points,social function(81.96±5.58)points and material life state(81.23±5.16)points in the comprehensive quality of life assessment questionnaire were higher than those in the control group,with statistical significance between the two groups(P<0.05).Conclusion High intensity intermittent cardiac rehabilitation training and external counterpulsation can improve cardiopulmonary fitness,cardiac function and quality of life in patients with coronary heart disease.
