醒脑开窍针刺法联合股直肌神经入肌点推拿点按疗法治疗中风后偏瘫的临床效果观察

Clinical Observation on the Effect of Xingnao Kaiqiao Acupuncture Combined with Massage and Point Pressing at the Point of Rectus Femoris Muscle Nerve Entering the Muscle on Post Stroke Hemiplegia

目的探讨醒脑开窍针刺法联合股直肌神经入肌点推拿点按疗法在中风后偏瘫中的应用效果。方法选取2020年1月—2022年12月我院收治的82例中风后偏瘫患者为研究对象,按随机数字表法分为对照组与观察组,各41例。对照组行股直肌神经入肌点推拿点按疗法治疗,观察组加用醒脑开窍针刺法治疗,持续12周。比较两组肢体运动功能、步行功能、Berg平衡量表(BBS)评分、Barthel指数评定量表(BI)评分、生活质量。结果治疗后,观察组Fugl-Meyer运动功能评分中上肢、下肢评分分别为(58.36±5.12)分、(22.58±2.76)分,均高于对照组,组间差异有统计学意义(P<0.05);观察组功能性步行分级量表、BBS、BI评分分别为(3.79±0.35)分、(42.65±4.63)分、(75.69±5.82)分,均高于对照组,10 m最大步行速度为(53.85±4.72)m/min,快于对照组,组间差异有统计学意义(P<0.05);观察组世界卫生组织生活简易量表评分中生理领域、心理领域、社会关系领域、环境领域评分分别为(86.41±5.47)分、(85.68±5.24)分、(87.88±5.16)分、(87.41±4.25)分,均高于对照组,组间差异有统计学意义(P<0.05)。结论醒脑开窍针刺法联合股直肌神经入肌点推拿点按可改善中风后偏瘫患者肢体运动功能及步行功能,提高生活质量。

Objective To investigate the application effect of Xingnaokaioqiao acupuncture combined with massage and point-pressing therapy of femoral rectus nerve entering muscle in patients with hemiplegia after stroke.Methods 82 patients with hemiplegia after stroke treated in our hospital from January 2020 to December 2022 were selected as the research objects and divided into a control group and an observation group according to random number table method,with 41 cases in each group.The control group was treated with massage and point-pressing therapy,and the observation group was treated with Xingnaokai acupuncture for 12 weeks.Limb motor function,walking function,Berg Balance Scale(BBS)score,Barthel Index rating scale(BI)score and quality of life were compared between the two groups.Results After treatment,the Fugl-Meyer motor function scores in the observation group were(58.36±5.12)points and(22.58±2.76)points,respectively,which were higher than those in the control group,and the differences were statistically significant(P<0.05);the scores of functional walking rating scale,BBS and BI of the observation group were(3.79±0.35)points,(42.65±4.63)points and(75.69±5.82)points,respectively,which were higher than those of the control group;the maximum walking speed at 10 m was(53.85±4.72)m/min,faster than that of the control group,the differences between groups were statistically significant(P<0.05);the scores of physiological field,psychological field,social relation field and environmental field in the observation group were(86.41±5.47)points,(85.68±5.24)points,(87.88±5.16)points and(87.41±4.25)points,respectively,which were higher than those of the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Xingnaokaiqiao acupuncture combined with the massage of femoral rectus nerve entering muscle point can improve the limb movement function and walking function of hemiplegic patients after stroke,and improve the quality of life.
