一指禅推拿下关、颊车穴联合中药外敷对颞颌关节紊乱病患者疼痛、最大主动开口度及炎性因子的影响

The Effect of Single Finger Zen Massage at Hsiakuan and Buccal Car Acupaints Combined with External Application of Traditional Chinese Medicine on Pain,MMO,and Inflammatory Factors in Patients with Temporomandibular Joint Disorders

目的探究一指禅推拿下关、颊车穴联合中药外敷对颞颌关节紊乱病(TMD)患者疼痛、最大主动开口度(MMO)及炎性因子的影响。方法选择本院2021年3月—2023年3月收治的80例TMD患者为研究对象,按照随机数字表法将其分为对照组与观察组,各40例。两组均采用超声波治疗,对照组采用一指禅推拿下关、颊车穴治疗,观察组在对照组的基础上联合中药外敷,两组均持续治疗2周。比较两组患者的临床疗效、疼痛程度、MMO及炎性因子水平。结果观察组的治疗总有效率为95.00%,高于对照组的77.50%,差异有统计学意义(P<0.05)。治疗前,两组疼痛程度、MMO及炎性因子水平比较,组间差异无统计学意义(P>0.05);治疗后,观察组疼痛视觉模拟评分低于对照组,MMO大于对照组,肿瘤坏死因子-α、白细胞介素-1β水平均低于对照组,转化生长因子-β1水平高于对照组,组间差异有统计学意义(P<0.05)。结论一指禅推拿下关、颊车穴联合中药外敷能够有效减轻TMD患者疼痛程度,增大MMO,降低炎性因子水平。

Objective To explore the effects of single finger zen massage at Hsiakuan and Bnccal car acupaints combined with external application of traditional Chinese medicine on pain,maximum range of motion(MMO),and inflammatory factors in patients with temporomandibular joint disorder(TMD).Methods 80 TMD patients admitted to our hospital from March 2021 to March 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 40 patients in each group.The two groups were treated with ultrasound,the control group was treated with one finger Zen massage at Hsiakuan and Bnccal car acupaints,the observation group was combined with traditional Chinese medicine on the basis of the control group,both groups were treated for 2 weeks.The clinical efficacy,pain degree,MMO and inflammatory factor levels were compared between the two groups.Results The total effective rate of the observation group was 95.00%,higher than 77.50%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in pain degree,MMO and inflammatory factors between the two groups(P>0.05);after treatment,the pain Visual Analogue Scale of the observation group was lower than that of the control group,the MMO was higher than that of the control group,the levels of tumor necrosis factor-αand interleukin-1βwere lower than those of the control group,and the levels of transforming growth factor-β1 were higher than those of the control group,the differences between groups were statistically significant(P<0.05).Conclusion Single finger Zen massage Hsiakuan and Buccal acupoints combined with external application of traditional Chinese medicine at Guanjiache point can effectively reduce the pain level of TMD patients,increase MMO,and reduce the level of inflammatory factors.
