改良序贯疗法联合益生菌治疗幽门螺杆菌阳性慢性萎缩性胃炎效果分析

Effect of Modified Sequential Therapy Combined with Probiotics in the Treatment of HP-Positive Chronic Atrophic Gastritis

目的探究改良序贯疗法联合益生菌对幽门螺杆菌(Hp)阳性慢性萎缩性胃炎效果.方法选取2021年6月—2022年12月本院收治的130例Hp阳性慢性萎缩性胃炎患者为研究对象,按照随机数字表法分为研究组(n=65)、对照组(n=65).对照组采用改良序贯疗法治疗,研究组在对照组基础上联合益生菌治疗,均治疗2周,比较两组疗效、Hp根除率、炎性因子、肠道菌群变化的差异.结果研究组治疗总有效率93.85%、Hp根除率90.77%均高于对照组,组间差异有统计学意义(P<0.05).治疗1周后、治疗2周后,研究组C反应蛋白、白细胞介素-6,白细胞介素-8、肿瘤坏死因子-α水平均低于对照组,组间差异有统计学意义(P<0.05).治疗2周后,研究组患者粪便中的双歧杆菌、乳酸杆菌含量及B/E值均高于对照组,肠杆菌、肠球菌含量均低于对照组,组间差异有统计学意义(P<0.05).结论改良序贯疗法联合益生菌治疗Hp阳性慢性萎缩性胃炎具有较好疗效,可改善患者炎性因子与肠道菌群.

Objective To investigate the effect of modified sequential therapy with probiotics on Helicobacter pylori(Hp)-positive chronic atrophic gastritis.Methods 130 patients with HP-positive chronic atrophic gastritis admitted to our hospital from June 2021 to December 2022 were prospectively selected as research objects,and were divided into study group(n=65)and control group(n=65)according to random number table method.The control group was treated with modified sequential therapy,and the research group was treated with probiotics on the basis of the control group for 2 weeks.The therapeutic effect,Hp eradication rate,inflammatory factors,intestinal flora changes were compared between the two groups.Results The total effective rate 93.85%and Hp eradication rate 90.77%of the study group were higher than those of the control group,with statistically significant differences(P<0.05).After 1 week and 2 weeks of treatment,the levels of C-reactive protein,interleukin-6,interleukin-8,and tumor necrosis factor-αin the study group were lower than those in the control group,with statistically significant differences(P<0.05).After 2 weeks of treatment,the levels of bifidobacteria and lactobacilli and B/E values of patients in the study group were higher than those in the control group,while the levels of Enterobacterium and Enterococcus were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion Modified sequential therapy combined with probiotics has better efficacy in treating HP-positive chronic atrophic gastritis,improves inflammatory factors and intestinal flora.
