新加皱肺汤对慢性阻塞性肺疾病稳定期患者膈肌功能及肺功能的影响

Effect of Xinjia Zhoufei Decoction on Diaphragm Function and Lung Function in Patients with Chronic Obstructive Pulmonary Disease in Stable Stage

目的探讨新加皱肺汤在慢性阻塞性肺疾病(COPD)稳定期患者中的应用效果.方法选取2020年6月—2022年6月本院收治的60例COPD稳定期患者为研究对象,按随机数字表法将其分为对照组和观察组,各30例.对照组采用吸入布地奈德福莫特罗治疗,观察组在对照组的基础上加用新加皱肺汤治疗,两组均持续治疗2周.比较两组患者的临床疗效、膈肌功能、肺功能指标、COPD评估测试(CAT)评分、mMRC评分及不良反应发生情况.结果治疗后,观察组治疗总有效率为93.33%,高于对照组的73.33%,呼气末膈肌厚度为(1.53±0.13)mm,薄于对照组的(1.72±0.15)mm,吸气末膈肌厚度为(3.25±0.31)mm,厚于对照组的(2.82±0.25)mm,膈肌移动度为(1.78±0.24)mm,大于对照组的(1.45±0.21)mm,最大吸气压为(70.36±6.24)cmH_(2)O(1 cmH_(2)O=0.098 kPa),高于对照组的(65.41±6.12)cmH_(2)O,组间差异有统计学意义(P<0.05);观察组血气指标优于对照组,差异有统计学意义(P<0.05);观察组用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))均大于对照组,FEV_(1)/FVC高于对照组,CAT评分、mMRC评分均低于对照组,组间差异有统计学意义(P<0.05);两组不良反应发生率相比,差异无统计学意义(P>0.05).结论新加皱肺汤可提高COPD稳定期的治疗效果,改善患者膈肌功能,提高肺通气能力,纠正血气异常,安全可靠.

Objective To investigate the application effect of Xinjia Zhoufei Decoction in stable chronic obstructive pulmonary disease(COPD)patients.Methods 60 with stable COPD treated in our hospital from June 2020 to June 2022 were selected as the study objects and divided into control group and observation group according to random number table method,with 30 patients in each group.The control group was treated with inhaled budesonide and formoterol,and the observation group was treated with Xinjia Zhoufei Decoction on the basis of the control group.Both groups were treated for 2 weeks.The clinical efficacy,diaphragm function,pulmonary function index,COPD evaluation test(CAT)score,mMRC score and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the observation group was 93.33%,higher than 73.33%of the control group,the diaphragm thickness at the end of expiratory breath was(1.53±0.13)mm,thinner than(1.72±0.15)mm of the control group,and the diaphragm thickness at the end of inspiratory breath was(3.25±0.31)mm,thicker than(2.82±0.25)mm of the control group,the movement degree of diaphragm was(1.78±0.24)mm,which was higher than(1.45±0.21)mm of the control group,and the maximum inspiratory pressure was(70.36±6.24)cmH_(2)O(1 cmH_(2)O=0.098 kPa),which was higher than(65.41±6.12)cmH_(2)O of control group,with statistical significance(P<0.05);the blood gas index of the observation group was better than that of the control group,the difference was statistically significant(P<0.05);forced vital capacity(FVC),forced expiratory volume at the first second(FEV_(1))in the observation group were greater than those in the control group,FEV_(1)/FVC was higher than that of the control group,and CAT score and mMRC score were lower than those in the control group,the differences between groups were statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Xinjia Zhoufei Decoction can improve the treatment effect of COPD stable stage,improve the diaphragm function of patients,improve lung ventilation capacity,correct abnormal blood gas,safe and reliable.
