温针灸肩三针联合中药离子导入治疗脑卒中后肩手综合征的效果观察

Observation on Therapeutic Effect of Shoulder-Hand Syndrome after Stroke Treated by Warming Needle Moxibustion Combined with Iontophoresis of Traditional Chinese Medicine

目的探讨温针灸肩三针联合中药离子导入治疗脑卒中后肩手综合征(SHS)的临床价值.方法选取2019年1月—2021年12月我院收治的90例脑卒中后SHS患者为研究对象,采用随机数表法将其均分为对照组和观察组.对照组采用常规康复治疗,观察组在此基础上加用温针灸肩三针联合中药离子导入,比较两组的治疗总有效率、疼痛视觉模拟(VAS)评分、Fugl-Meyer运动功能评定量表(FMA)评分、中医证候积分与肩关节活动度.结果观察组的治疗总有效率为95.56%,高于对照组的82.22%,差异有统计学意义(P<0.05).治疗后,观察组FMA评分高于对照组,肩关节活动度大于对照组,VAS评分、中医症候积分均低于对照组,组间差异有统计学意义(P<0.05).结论在SHS康复治疗期间,温针灸肩三针联合中药离子导入具有良好的治疗效果,可提高患者的治疗有效率,促进镇痛与肢体功能恢复,降低中医症候积分.

Objective To investigate the clinical value of warm acupuncture shoulder three acupuncture combined with Chinese medicine ion introduction in the treatment of post-stroke shoulder-hand syndrome(SHS).Methods 90 patients with SHS after stroke who were admitted to our hospital from January 2019 to December 2021 were selected as the research objects,and they were divided into control group and observation group by the random number table method.The control group was treated with routine rehabilitation treatment,and the observation group was additionally treated with warm acupuncture and three needles on the shoulder combined with traditional Chinese medicine iontophoresis.The total effective rate,pain Visual Analogue Score(VAS),Fugl-Meger Assessment Score,TCM syndrome score and shoulder joint activity were compared between the two groups.Results The total effective rate of the observation group was 95.56%,which was higher than 82.22%of the control group,and the difference was statistically significant(P<0.05).After treatment,the FMA score and range of motion of shoulder joint in the observation group were higher than those in the control group,and the VAS Score and TCM syndrome score were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusion During the rehabilitation treatment of SHS,warm acupuncture shoulder three acupuncture combined with Chinese medicine ion introduction has good therapeutic effect,which can improve the treatment efficiency of patients,promote analgesia and limb function recovery,and reduce the Chinese medicine symptom score.
