黄芪活骨颗粒联合改良空心钉内固定术治疗中青年股骨颈骨折的临床效果

Clinical Effect of Huangqi Huogu Granule Combined with Modified Internal Fixation with Closed Reduction and Cannulated Screws in the Treatment of Femoral Neck Fracture in Young and Middle-aged Patients

目的探析黄芪活骨颗粒联合改良闭合复位空心钉内固定术治疗中青年股骨颈骨折的临床效果。方法选取2018年6月—2021年8月日照市中医医院收治的67例中青年股骨颈骨折患者为研究对象,随机分为两组。对照组32例采用黄芪活骨颗粒联合传统空心钉内固定术治疗,观察组35例采用黄芪活骨颗粒联合改良空心钉内固定术治疗。比较两组患者的围术期指标、随访指标及临床疗效。结果观察组的手术时间长于对照组,术中出血量少于对照组,切口长度短于对照组,组间差异有统计学意义(P<0.05)。观察组的骨折愈合时间短于对照组,末次随访时HHS评分高于对照组,晚期并发症发生率低于对照组,组间差异有统计学意义(P<0.05)。观察组的治疗总显效率为94.29%,高于对照组的78.13%,差异有统计学意义(P<0.05)。结论采用黄芪活骨颗粒联合改良空心钉内固定术治疗中青年股骨颈骨折的效果良好,可缩短患者骨折愈合时间,改善其髋关节功能,降低并发症发生风险,值得临床推广使用。

Objective To explore the clinical effect of Huangqi Huogu Granule combined with modified internal fixation with closed reduction and cannulated screws in the treatment of femoral neck fracture in young and middle-aged patients.Methods 67 middle-aged and young patients with femoral neck fracture admitted to Rizhao Hospital of Traditional Chinese Medicine from June 2018 to August 2021 were selected as the research objects and randomly divided into two groups.32 cases in the control group were treated with Huangqi Huogu Granule combined with traditional internal fixation with closed reduction and cannulated screws,and 35 cases in the observation group were treated with Huangqi Huogu Granule combined with modified internal fixation with closed reduction and cannulated screws.The perioperative indexes,follow-up indexes and clinical efficacy of the two groups were compared.Results The operation time of the observation group was longer than that of the control group,the amount of bleeding during operation was less than that of the control group,and the incision length was shorter than that of the control group,the differences between the groups were statistically significant(P<0.05).The fracture healing time of the observation group was shorter than that of the control group,the Harris hip score at the last follow-up was higher than that of the control group,and the incidence of late complications was lower than that of the control group,the differences between the groups were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 94.29%,higher than 78.13%in the control group,and the difference was statistically significant(P<0.05).Conclusion Huangqi Huogu Granule combined with modified internal fixation with closed reduction and cannulated screws in the treatment of middle-aged and young femoral neck fractures has good effect,can shorten the fracture healing time of patients,improve the hip function,reduce the risk of complications,and is worthy of clinical promotion.
