腹腔镜下胃癌根治术后发生低蛋白血症的相关影响因素分析

Analysis of Related Influencing Factors of Hypoproteinemia after Laparoscopic Radical Gastrectomy for Gastric Cancer

目的分析腹腔镜下胃癌根治术后发生低蛋白血症的相关影响因素。方法选择2020年1月—2022年12月于威海口腔医院外科行腹腔镜下胃癌根治术的116例患者为研究对象,根据术后是否发生低蛋白血症分为低蛋白血症组、非低蛋白血症组。比较两组患者的资料,分析腹腔镜下胃癌根治术后发生低蛋白血症的相关危险因素。结果116例腹腔镜下胃癌根治术患者中,37例发生低蛋白血症,低蛋白血症发生率为31.90%。经Logistic回归分析结果显示,肿瘤最大径>5 cm、手术时间>5 h、术前血红蛋白水平<120 g/L、术前血清白蛋白水平<35 g/L是腹腔镜下胃癌根治术后发生低蛋白血症的危险因素(OR>1,P<0.05)。结论腹腔镜下胃癌根治术患者术后发生低蛋白血症的风险较高,其危险因素包括肿瘤最大径>5 cm、手术时间>5 h、术前血红蛋白水平<120 g/L、术前血清白蛋白水平<35 g/L。

Objective To analyze the related influencing factors of hypoproteinemia after laparoscopic radical gastrectomy for gastric cancer.Methods 116 patients who underwent laparoscopic radical gastrectomy in Weihai Stomatological Hospital from January 2020 to December 2022 were selected as the study objects.They were divided into a hypoproteinemia group and a non-hypoproteinemia group according to whether they had hypoproteinemia after surgery.The data of the two groups were compared to analyze the risk factors of hypoproteinemia after laparoscopic radical gastrectomy.Results Among 116 patients undergoing laparoscopic radical gastrectomy,37 cases had hypoproteinemia,the incidence of hypoproteinemia was 31.90%.Logistic regression analysis showed that maximum tumor diameter>5 cm,operation time>5 h,preoperative hemoglobin level<120 g/L,and preoperative serum albumin level<35 g/L were risk factors for hypoproteinemia after laparoscopic radical gastrectomy(OR>1,P<0.05).Conclusion Gastric cancer patients undergoing laparoscopic radical gastrectomy have a higher risk of hypoproteinemia after surgery,and its risk factors include maximum tumor diameter>5 cm,operation time>5 h,preoperative hemoglobin level<120 g/L,and preoperative serum albumin level<35 g/L.
