中药熏蒸联合平衡功能强化训练对踝关节骨折术后患者关节肿痛及踝关节功能的影响

Effects of Traditional Chinese Medicine Fumigation Combined with Balance Function Strengthening Training on Joint Swelling and Pain and Ankle Function in Patients after Ankle

目的探讨中药熏蒸联合平衡功能强化训练对踝关节骨折术后患者关节肿痛及踝关节功能的影响。方法选取2019年4月—2022年4月我院收治的98例踝关节骨折术后患者为研究对象,随机分为观察组(n=49)与对照组(n=49)。对照组采用平衡功能强化训练,观察组采用中药熏蒸联合平衡功能强化训练。对比两组的关节肿痛、踝关节功能、炎性因子水平。结果治疗前,两组踝关节周径、疼痛视觉模拟(VAS)评分对比,组间差异无统计学意义(P>0.05);治疗60 d后,两组踝关节周径均较同组治疗前缩短、VAS评分均较同组治疗前降低,且观察组踝关节周径短于对照组、VAS评分低于对照组,组间差异有统计学意义(P<0.05)。治疗前,两组Baird-Jackson踝关节评分、足踝能力评估量表(FAAM)评分、美国足踝外科医师协会(AOFAS)踝—后足评分对比,组间差异无统计学意义(P>0.05);治疗60 d后,两组Baird-Jackson踝关节评分、FAAM评分、AOFAS踝—后足评分均较同组治疗前升高,且观察组Baird-Jackson踝关节评分、FAAM评分、AOFAS踝—后足评分均高于对照组,组间差异有统计学意义(P<0.05)。治疗前,两组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)水平对比,组间差异无统计学意义(P>0.05);治疗60 d后,两组TNF-α、IL-6、IL-1β水平均较同组治疗前降低,且观察组TNF-α、IL-6、IL-1β水平均低于对照组,组间差异有统计学意义(P<0.05)。结论中药熏蒸联合平衡功能强化训练能减轻踝关节骨折术后患者关节肿痛,促进踝关节功能恢复,减轻炎症反应。

Objective To investigate the effect of traditional Chinese medicine fumigation combined with balance function strengthening training on ankle joint swelling and pain and ankle joint function in patients after ankle fracture surgery.Methods 98 patients after ankle fracture surgery admitted to the hospital from April 2019 to April 2022 were selected as the research objects and randomly divided into an observation group(n=49)and a control group(n=49).The control group used balance function intensive training,and the observation group used traditional Chinese medicine fumigation combined with balance function intensive training.Joint swelling and pain,ankle function and the levels of inflammatory factors were compared between the two groups.Results Before treatment,there were no significant differences in circumference diameter of ankle and pain visual analogue scale(VAS)scores between the two groups(P>0.05);after 60 days of treatment,the circumference diameters of ankle in both groups were shorter than those before treatment,the VAS scores in both groups were lower than those before treatment,and the circumference diameter of ankle of the observation group was shorter than that of the control group,and the VAS score was lower than that of the control group,the differences between the two groups were statistically significant(P<0.05).Before treatment,Baird-Jackson ankle score,foot and ankle ability measure(FAAM)score and American orthopaedic foot and ankle society(AOFAS)ankle-posterior foot score were compared between the two groups,and there was no significant difference between the two groups(P>0.05);after 60 days of treatment,Baird-Jackson ankle score,FAAM score and AOFAS ankle-posterior foot score in both groups were higher than before treatment,and the Baird-Jackson ankle score,FAAM score and AOFAS ankle-posterior score in the observation group were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Before treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-1β(IL-1β)were compared between the two groups,and there was no significant difference between the two groups(P>0.05);after 60 days of treatment,the levels of TNF-α,IL-6 and IL-1β in two groups were decreased compared with those before treatment,and the levels of TNF-α,IL-6 and IL-1β in the observation group were lower than those in control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Traditional Chinese medicine fumigation combined with balance function strengthening training can reduce the swelling and pain of joint in patients after ankle fracture surgery,promote the recovery of ankle function,and reduce inflammatory reaction.
