三间隙引流术与传统切开挂线术对肛周脓肿患者治疗效果及肛门功能与疼痛程度的影响

The Effect of Three-space Drainage and Traditional Incision on the Treatment Effect,Anal Function and Pain Degree in Patients with Perianal Abscess

目的分析三间隙引流术与传统切开挂线术对肛周脓肿患者治疗效果及肛门功能、疼痛程度的影响.方法选择我院2020年3月—2022年9月收治的68例肛周脓肿患者为研究对象,按随机数字表法分成两组.对照组(n=34)行传统切开挂线术,观察组(n=34)行三间隙引流术.对比两组患者的临床疗效、围术期指标、肛门功能、疼痛程度与术后并发症.结果观察组的治疗总有效率为97.06%,高于对照组的76.48%,差异有统计学意义(P<0.05).两组手术时间比较,差异无统计学意义(P>0.05).观察组术中出血量(18.62±3.49)mL少于对照组的(24.36±4.58)mL,住院时间(6.26±0.93)d短于对照组的(7.34±1.28)d,组间差异有统计学意义(P<0.05).术后观察组便秘评分系统(3.68±0.72)分、疼痛视觉模拟评分(3.18±0.58)分,均低于对照组的(4.97±1.06)分、(4.52±0.84)分,组间差异有统计学意义(P<0.05).观察组术后并发症发生率5.88%低于对照组的23.52%,差异有统计学意义(P<0.05).结论相较于采用传统切开挂线术治疗肛周脓肿患者,三间隙引流术可进一步提高临床疗效,改善肛门功能,有效缓解疼痛,且术后并发症少,安全性更高.

Objective To analyze the effect of triple space drainage and traditional open wire hanging surgery on the treatment effect,anal function and pain degree in patients with perianal abscess.Methods 68 patients with perianal abscess admitted to our hospital from March 2020 to September 2022 were selected as the research objects and divided into two groups according to the random number table method.The control group(n=34)performed conventional incision and hanging,and the observation group(n=34)performed three-gap drainage.The clinical efficacy,perioperative indicators,anal function,pain degree,and postoperative complications were compared between the two groups.Results The total effective rate of the observation group was 97.06%,higher than 76.48%of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).The intraoperative blood loss in the observation group(18.62±3.49)mL was less than that in the control group(24.36±4.58)mL,and the length of hospital stay(6.26±0.93)d was shorter than(7.34±1.28)d in the control group,the difference was statistically significant between the two groups(P<0.05).Postoperative constipation score system score(3.68±0.72)points and visual analog pain score(3.18±0.58)points in the observation group,were lower than(4.97±1.06)points and(4.52±0.84)points in the control group,respectively,and the differences were statistically significant between the two groups(P<0.05).The complication rate of the observation group was 5.88%lower than 23.52%of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with the traditional treatment for patients with perianal abscess,three-space drainage can further improve the clinical efficacy,improve anal function,effectively relieve pain,and have fewer postoperative complications and higher safety.
