三间隙引流术对肛周脓肿患者术后疼痛与肛门功能的影响

Effect of Three-Space Drainage on Postoperative Pain,Anal Function in Patients with Perianal Abscess

目的探讨三间隙引流术对肛周脓肿患者术后疼痛与肛门功能的影响.方法选取2018年10月—2021年11月该院收治的118例肛周脓肿患者为研究对象,采用随机数字表法分为两组,各59例,对照组采用传统切开挂线术治疗,观察组采用三间隙引流术治疗,比较两组的临床指标、疼痛视觉模拟(VAS)评分、便秘评分量表(Wexner)评分及并发症发生情况.结果与对照组比较,观察组术中出血量较少,创面愈合时间与住院时间均较短,观察组术后7、30d的VAS评分、术后1d的Wexner评分均较低,且观察组术后并发症发生率1.69%,低于对照组的13.56%,组间差异有统计学意义(P<0.05).结论与传统切开挂线术比较,三间隙引流术效果更明显,肛周脓肿患者术中出血量较少,恢复时间较短,且术后疼痛感缓解较快,不会对肛门功能造成明显影响,具有一定安全性.

Objective To investigate the effects of three-space drainage on postoperative pain,anal function in patients with perianal abscess.Methods 118 patients with perianal abscess admitted to our hospital from October 2018 to November 2021 were selected as the study objects and divided into two groups with 59 cases in each group by random number table method.The control group was treated with traditional incision and hanging suture,and the observation group was treated with three-space drainage.The clinical indexes,visual analogue pain(VAS)score,constipation Rating Scale(Wexner)and complications of the two groups were compared.Results Compared with the control group,the observation group had less intraoperative blood loss,shorter wound healing time and shorter hospital stay,lower VAS score at 7 and 30 days after surgery and Wexner score at 1 day after surgery,and the incidence of postoperative complications in the observation group was 1.69%,which was lower than 13.56%in the control group,the differences between groups were statistically significant(P<0.05).Conclusion Compared with traditional incision and thread drawing,the effect of three gap drainage is more obvious.Patients with perianal abscess have less intraoperative blood loss,shorter recovery time,faster postoperative pain relief,and will not cause significant impact on anal function,which has a certain degree of safety.
