黄体酮联合妈富隆对围绝经期功能失调性子宫出血患者血清性激素及子宫内膜的影响

Effects of Progesterone Combined with Marvelon on Serum Sex Hormones and Endometrium in Patients with Dysfunctional Uterine Bleeding in Perimenopausal Period

目的探究黄体酮联合妈富隆对围绝经期功能失调性子宫出血(DUB)患者血清性激素及子宫内膜的影响.方法选择2021年1月—2022年11月本院收治的62例围绝经期DUB患者为研究对象,按随机数字表法分为两组.对照组(n=31)采用黄体酮胶囊治疗,观察组(n=31)采用黄体酮联合妈富隆治疗,均治疗3个疗程.对比两组患者的临床疗效、血清性激素水平、子宫内膜厚度、子宫平滑肌收缩幅度及不良反应发生情况.结果观察组的治疗总有效率为96.77%,高于对照组的74.19%,差异有统计学意义(P<0.05).治疗后,观察组的卵泡刺激素、黄体生成素、雌二醇水平分别为(24.41±6.52)U/L、(21.54±4.33)U/L、(132.34±14.35)pmol/L,均低于对照组的(35.84±7.65)U/L、(30.82±7.16)U/L、(256.74±16.72)pmol/L,组间差异有统计学意义(P<0.05).治疗后,观察组的子宫内膜厚度为(4.11±0.51)mm,薄于对照组的(5.16±0.73)mm,子宫平滑肌收缩幅度为(9.07±0.84)g,大于对照组的(8.01±0.68)g,组间差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论围绝经期DUB患者采用黄体酮联合妈富隆治疗的效果理想,能有效降低子宫内膜厚度,调节性激素水平,提高子宫平滑肌收缩幅度,且安全性高.

Objective To investigate the effects of progesterone combined with Marvelon on serum sex hormones and endometrium in patients with perimenopausal dysfunctional uterine bleeding(DUB).Methods 62 patients with perimenopausal DUB admitted to the hospital from January 2021 to November 2022 were selected as the study objects and divided into two groups according to random number table method.The control group(n=31)was treated with progesterone capsule,and the observation group(n=31)was treated with progesterone combined with Marvelon.Both groups were treated for 3 courses.The clinical efficacy,serum sex hormone levels,endometrial thickness,uterine smooth muscle contraction amplitude and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 96.77%,which was higher than 74.19%of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of follicle stimulating hormone,luteinizing hormone and estradiol in the observation group were(24.41±6.52)U/L,(21.54±4.33)U/L and(132.34±14.35)pmol/L,respectively,which were lower than(35.84±7.65)U/L,(30.82±7.16)U/L,(256.74±16.72)pmol/L in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the endometrial thickness in the observation group was(4.11±0.51)mm,thinner than(5.16±0.73)mm in the control group,and the contraction amplitude of uterine smooth muscle was(9.07±0.84)g,higher than(8.01±0.68)g in the control group,the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion In perimenopausal DUB patients,progesterone combined with Marvelon can effectively reduce endometrial thickness,regulate sex hormone levels,and increase the contraction amplitude of uterine smooth muscle,with high safety.
