摘要
目的探讨机器人辅助下微创立体定向软通道置管血肿引流术(简称“微创术”)治疗高血压基底节区出血的效果。方法选择2021年2月—2022年11月就诊于济南市第三人民医院的70例高血压基底节区出血患者为研究对象,根据随机数字表法分为对照组及治疗组,每组35例。两组均采用微创手术治疗,对照组手术在CT定位下操作,治疗组手术在机器人辅助下操作。对比两组患者的手术相关指标、日常生活能力、神经功能及并发症发生情况。结果两组的血肿清除率比较,差异无统计学意义(P>0.05)。治疗组的住院时间、手术时间均短于对照组,术后引流量少于对照组,组间差异有统计学意义(P<0.05)。术后3个月,治疗组的日常生活能力评分高于对照组,美国国立卫生研究院卒中量表评分低于对照组,组间差异有统计学意义(P<0.05)。治疗组的并发症发生率为5.71%,低于对照组的22.86%,差异有统计学意义(P<0.05)。结论采用机器人辅助下微创术治疗高血压基底节区出血的效果理想,能减少并发症发生,促进患者神经功能改善,提高其日常生活能力。
Objective To investigate the effect of robot-assisted minimally invasive stereotactic soft channel catheterization and hematoma drainage(referred to as"microwound surgery")in the treatment of hypertensive basal ganglia hemorrhage.Methods 70 patients with hypertensive basal ganglia hemorrhage admitted to Jinan Third People's Hospital from February 2021 to November 2022 were selected as the study objects and were divided into a control group and a treatment group according to random number table method,with 35 cases in each group.Both groups received minimally invasive surgery,the control group operated with CT localization,the treatment group operated with robot-assisted.The operation related indexes,daily living ability,nerve function and complications were compared between the two groups.Results There was no significant difference in the clearance rate of hematoma between the two groups(P>0.05).The hospitalization time and operation time of the treatment group were shorter than those of the control group,and the postoperative drainage volume was fewer than that of the control group,the differences between the groups were statistically significant(P<0.05).After 3 months of operation,the activities of daily living score of the treatment group was higher than that of the control group,and the National Institute of Health Stroke Scale score was lower than that of the control group,the differences between the groups were statistically significant(P<0.05).The incidence of complications in the treatment group was 5.71%,lower than 22.86%in the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of robot-assisted microwound surgery in the treatment of hypertensive basal ganglia hemorrhage is ideal,which can reduce the occurrence of complications,promote the improvement of nerve function and improve the ability of daily life.
