主动循环呼吸技术训练联合肺康复运动训练对胸腔镜下肺叶切除术后患者康复效果的影响

The Influence of Active Circulatory Breathing Technique Training Combined with Pulmonary Rehabilitation Exercise Training on Rehabilitation Effect of Patients after Thoracoscopic Lobectomy

摘要 目的探讨主动循环呼吸技术训练联合肺康复运动训练对胸腔镜下肺叶切除术后患者康复效果的影响。方法选取2021年2月—2023年2月于南京医科大学附属脑科医院行胸腔镜下肺叶切除术的90例患者为研究对象,根据不同干预方案分为对照组及研究组。对照组(n=45)进行肺康复运动训练,研究组(n=45)在对照组基础上进行主动循环呼吸技术训练。比较两组患者的肺功能、生活质量、术后恢复效果及并发症发生情况。结果干预后,研究组的第1秒用力呼气容积、用力肺活量、最大肺活量均高于对照组,峰值呼气流速快于对照组,组间差异有统计学意义(P<0.05)。干预后,研究组的肺癌FACT-L量表中各维度评分及总分均高于对照组,组间差异有统计学意义(P<0.05)。研究组的保留引流管时间、术后首次排痰时间及住院时间均短于对照组,组间差异有统计学意义(P<0.05)。研究组的并发症发生率为6.67%,低于对照组的22.22%,差异有统计学意义(P<0.05)。结论主动循环呼吸技术训练联合肺康复运动训练有助于减少胸腔镜下肺叶切除术后患者的肺部并发症,缩短康复时间,促进肺功能恢复,改善其生活质量。 Objective To investigate the effect of active circulatory breathing technique training combined with pulmonary rehabilitation exercise training on rehabilitation of patients after thoracoscopic lobectomy.Methods 90 patients who underwent thoracoscopic lobectomy in Brain Hospital Affiliated to Nanjing Medical University from February 2021 to February 2023 were selected as the study subjects and were divided into a control group and a study group according to different intervention plans.The control group(n=45)received pulmonary rehabilitation exercise training,and the study group(n=45)received active circulation breathing technique training on the basis of the control group.The pulmonary function,quality of life,postoperative recovery and complications were compared between the two groups.Results After intervention,the forced expiratory volume in one second,forced vital capacity and maximum vital capacity of the study group were higher than those of the control group,peak expiratory flow rate faster than the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the scores and total scores of lung cancer FACT-L scale in the study group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The retention time of drainage tube,the first postoperative expectoration time and the length of hospital stay in the study group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of complications in the study group was 6.67%,lower than 22.22%in the control group,and the difference was statistically significant(P<0.05).Conclusion Active circulatory breathing training combined with pulmonary rehabilitation exercise training can reduce the pulmonary complications,shorten the rehabilitation time,promote the recovery of pulmonary function and improve the quality of life of patients after thoracoscopic lobectomy.
