卡前列甲酯栓联合麦角新碱对产后出血患者出血量及子宫复旧的影响

Effect of Methyl Carboprost Suppositories Combined with Ergometrine on Blood Loss and Uterine Relapse in Patients with Postpartum Hemorrhage

目的探究卡前列甲酯栓联合麦角新碱对产后出血患者出血量及子宫复旧的影响.方法选择本院2021年5月—2022年6月收治的86例产后出血患者为研究对象,按随机数字表法分为对照组、观察组,每组43例.对照组采用卡前列甲酯栓治疗,观察组在此基础上采用麦角新碱治疗.比较两组患者的出血量、子宫复旧情况、凝血指标及安全性.结果观察组产后2、24 h的出血量分别为(247.12±16.37)mL、(302.73±23.69)mL,均少于对照组的(304.65±22.35)mL、(398.33±20.26)mL,组间差异有统计学意义(P<0.05).观察组产后第3、5天的宫底高度均低于对照组,组间差异有统计学意义(P<0.05).用药后,观察组的凝血活酶时间、凝血酶凝固时间、凝血酶原时间均短于对照组,纤维蛋白原水平高于对照组,组间差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论采用卡前列甲酯栓联合麦角新碱治疗产后出血,能有效减少患者出血量,降低其宫底高度,改善凝血功能,安全可靠,值得临床推广使用.

Objective To investigate the effects of methyl carbprost suppositant combined with ergometrine on blood loss and uterine relapse in patients with postpartum hemorrhage.Methods 86 patients with postpartum hemorrhage admitted to the hospital from May 2021 to June 2022 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 43 cases in each group.The control group was treated with methyl carbprost suppositant,and the observation group was treated with ergometrine on this basis.The blood loss,uterine involution,coagulation indexes and safety of the two groups were compared.Results The postpartum blood loss of 2 and 24 h in the observation group were(247.12±16.37)mL and(302.73±23.69)mL,respectively,which were less than(304.65±22.35)mL and(398.33±20.26)mL in the control group,and the differences between the groups were statistically significant(P<0.05).The fundal heights of the observation group were lower than those of the control group on the 3rd and 5th day after delivery,and the differences between the groups were statistically significant(P<0.05).After treatment,the thrombin time,thrombin coagulation time and prothrombin time in observation group were shorter than those in control group,and the fibrinogen level was higher than that in control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Methyl carbprost suppositant combined with ergometrine in the treatment of postpartum hemorrhage can effectively reduce the amount of blood loss,reduce the height of the uterine fundus,improve the coagulation function,and is safe and reliable,worthy of clinical promotion.
