达格列净联合骨化三醇对糖尿病肾病患者炎症因子及尿蛋白水平的影响

Effects of Dapagliflozin Combined with Calcitriol on Inflammatory Factors and Urinary Protein Levels in Patients with Diabetes Nephropathy

目的探讨达格列净联合骨化三醇对糖尿病肾病(DN)患者炎症因子及尿蛋白水平的影响.方法选取本院2019年7月—2021年7月收治的80例DN患者为研究对象,根据随机数字表法分为对照组和观察组.对照组(n=40)采用达格列净治疗,观察组(n=40)在此基础上采用骨化三醇治疗.比较两组患者的临床疗效、炎症因子水平、糖化血红蛋白水平及尿蛋白水平.结果观察组的治疗总有效率为95.00%,高于对照组的75.00%,差异有统计学意义(P<0.05).治疗后,观察组的C反应蛋白、肿瘤坏死因子-α、白细胞介素-6及糖化血红蛋白水平均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的转铁蛋白、a1-微球蛋白、免疫球蛋白G水平均低于对照组,组间差异有统计学意义(P<0.05).结论达格列净联合骨化三醇治疗可降低DN患者的炎症水平及尿蛋白水平,改善其肾功能,提高临床疗效,值得临床推广使用.

Objective To investigate the effect of dapagliflozin combined with calcitriol on inflammatory factors and urinary protein levels in patients with diabetes nephropathy(DN).Methods 80 DN patients admitted to the hospital from July 2019 to July 2021 were selected as the study objects,and were divided into a control group and an observation group according to random number table method.The control group(n=40)was treated with dapagliflozin,and the observation group(n=40)was treated with calcitriol on this basis.The clinical efficacy,inflammatory factor levels,glycosylated hemoglobin level and urinary protein levels were compared between the two groups.Results The total effective rate of the observation group was 95.00%,which was higher than 75.00%of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of C-reactive protein,tumor necrosis factor-α,interleukin-6 and haemoglobin A1C in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of transferrin,α1-microglobulin and immunoglobulin G in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Dapagliflozin combined with calcitriol can reduce the level of inflammation and urinary protein in patients with DN,improve renal function and clinical efficacy,which is worthy of clinical promotion.
