参苓汤联合饮食护理对脾肾气虚型原发性膜性肾病患者生化指标及生活质量的影响

Effect of Shenling Decoction Combined with Diet Nursing on Biochemical Indexes and Quality of Life in Patients with Primary Membranous Nephropathy of Spleen Kidney Qi Deficiency Type

目的探讨参苓汤联合饮食护理对脾肾气虚型原发性膜性肾病患者生化指标及生活质量的影响.方法选择2021年1月—2022年6月玉林市中医医院肾病科收冶的80例脾肾气虚型原发性膜性肾病患者为研究对象,根据随机数字表法分为两组,每组40例.对照组采用常规饮食护理,观察组在此基础上采用参苓汤干预.比较两组患者的生化指标、生活质量、中医症候积分、干预效果及不良反应发生情况.结果干预后,观察组的血清白蛋白水平高于对照组,24 h尿蛋白定量低于对照组,组间差异有统计学意义(P<0.05);两组的总胆固醇、甘油三酯、低密度脂蛋白、血钾及血肌酐水平比较,组间差异无统计学意义(P>0.05).干预后,观察组的世界卫生组织生活质量评定量表简表中各维度评分均高于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的中医证候积分为(4.67±1.89)分,低于对照组的(7.84±2.06)分,差异有统计学意义(P<0.05).观察组的干预总有效率为97.50%,高于对照组的65.00%,差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论参苓汤联合饮食护理可改善脾肾气虚型原发性膜性肾病患者的血清白蛋白、24 h尿蛋白定量水平,提高其生活质量,降低中医证候积分,干预效果理想,且安全性较高,值得临床推广使用.

Objective To investigate the effects of Shenling Decoction combined with diet nursing on biochemical indexes and quality of life in patients with primary membranous nephropathy of spleen kidney QI deficiency type.Methods 80 patients with primary membranous nephropathy of spleen kidney Qi deficiency type admitted to the Nephrology Department of Yulin Hospital of Traditional Chinese Medicine from January 2021 to June 2022 were selected as the research objects and were divided into two groups according to random number table method,with 40 cases in each group.The control group was treated with routine diet nursing,and the observation group was treated with Shenling Decoction on this basis.The biochemical indexes,quality of life,TCM symptom score,intervention effect and adverse reactions were compared between the two groups.Results After intervention,the serum albumin level in the observation group was higher than that in the control group,and the 24 h urinary protein quantity was lower than that in the control group,the differences between the groups were statistically significant(P<0.05);there were no significant differences in the levels of total cholesterol,triglyceride,low density lipoprotein,serum potassium and serum creatinine between the two groups(P>0.05).After intervention,the scores of all dimensions in the World Health Organization quality of life-BREF scale in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the TCM syndrome score of the observation group was(4.67±1.89)points,which was lower than(7.84±2.06)points of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was 97.50%,higher than 65.00%of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Shenling Decoction combined with diet nursing can improve the quantitative levels of serum albumin and 24 h urinary protein,improve the quality of life and reduce the TCM syndrome score in patients with primary membrane nephropathy of spleen kidney Qi deficiency type.The intervention effect is ideal,and the safety is high,which is worthy of clinical promotion.
