火龙罐联合温针灸对脑卒中后肩手综合征患者患肢疼痛、肿胀及运动功能的影响

Effect of Huolong Pot Combined with Warm Acupuncture on Pain,Swelling and Motor Function of Affected Limbs in Patients with Shoulder Hand Syndrome after Stroke

目的探究火龙罐联合温针灸治疗脑卒中后肩手综合征(SHS)对患者患肢疼痛、肿胀及运动功能的影响.方法选取2020年3月—2021年3月本院收治的84例脑卒中后SHS患者为研究对象,按随机数字表法分为两组,每组42例.对照组采用温针灸治疗,观察组采用火龙罐联合温针灸治疗,均持续治疗4周.对比两组治疗前后的患肢疼痛视觉模拟(VAS)评分、肿胀程度、Fugl-Meyer上肢运动功能评定量表(FMA-UE)评分及肩手综合征评分量表(SHSS)评分.结果治疗后,观察组的患肢VAS评分为(2.79±0.47)分,低于对照组的(4.15±0.60)分,肿胀程度为(7.59±0.62)mL,小于对照组的(9.42±1.83)mL,组间差异有统计学意义(P<0.05).治疗后,观察组的FMA-UE评分为(45.28±4.11)分,高于对照组的(37.87±3.50)分,SHSS评分为(3.47±0.76)分,低于对照组的(5.03±0.89)分,组间差异有统计学意义(P<0.05).结论采用火龙罐联合温针灸治疗脑卒中后SHS,能有效减轻患肢疼痛、肿胀,改善患者上肢运动功能及临床症状,值得临床推广使用.

Objective To explore the effects of the treatment of post-stroke shoulder hand syndrome(SHS)with Huolong Pot combined with warm acupuncture on the pain,swelling and motor function of the affected limbs.Methods A total of 84 patients with post-stroke SHS admitted to our hospital from March 2020 to March 2021 were selected as the research objects and were divided into two groups according to random number table method,with 42 cases in each group.The control group was treated with warm acupuncture,and the observation group was treated with Huolong Pot combined with warm acupuncture.Both groups were treated for 4 weeks.The pain visual analog scale(VAS)score and swelling degree of affected limbs,Fugl-Meyer assessment upper extremity scale(FMA-UE)score and shoulder hand syndrome score(SHSS)score in both groups were compared before and after treatment.Results After treatment,the VAS score of the observation group was(2.79±0.47)points,which was lower than(4.15±0.60)points of the control group,and the swelling degree was(7.59±0.62)mL,which was lower than(9.42±1.83)mL of the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the FMA-UE score of the observation group was(45.28±4.11)points,higher than(37.87±3.50)points of the control group,and the SHSS score was(3.47±0.76)points,lower than(5.03±0.89)points of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion The combination of Huolong Pot and warm acupuncture in the treatment of post-stroke SHS can effectively reduce the pain and swelling of the affected limb,improve the motor function of the upper limb and clinical symptoms,and is worthy of clinical promotion.
