桥接组合式内固定系统治疗不稳定性肩胛骨骨折的疗效观察

Clinical Observation on the Treatment of Unstable Scapular Fracture by Ortho-Bridge System

目的观察桥接组合式内固定系统(OBS)治疗不稳定性肩胛骨骨折(USF)的效果.方法选取2018年3月—2022年9月于我院接受手术治疗的62例USF患者为研究对象,根据手术方法不同分为两组,每组31例.OBS组采用OBS治疗,重建钢板(RP)组采用RP治疗.比较两组患者的围术期指标、住院时间、主动活动时间、完全负重时间及肩关节功能.结果OBS组的手术时间、住院时间均短于RP组,术中出血量、术后引流量均少于RP组,术中透视次数少于RP组,组间差异有统计学意义(P<0.05).OBS组的主动活动时间及完全负重时间分别为(4.93±0.65)d、(55.31±7.13)d,均短于RP组的(10.37±2.65)d、(82.33±9.93)d,组间差异有统计学意义(P<0.05).术后2周,OBS组的Constant-Murley肩关节评分高于RP组,前屈上举活动度、外展上举活动度均大于RP组,组间差异有统计学意义(P<0.05).结论与RP相比,采用OBS治疗USF患者可改善围术期指标及肩关节功能,减少住院时间,值得临床推广使用.

Objective To observe the effect of Ortho-bridge system(OBS)in the treatment of unstable scapular fracture(USF).Methods 62 patients with USF who received surgical treatment in the hospital from March 2018 to September 2022 were selected as the research objects and were divided into two groups according to different surgical methods,with 31 cases in each group.The OBS group was treated with OBS,while the reconstruction plate(RP)group was treated with RP.The perioperative indexes,length of hospital stay,active time,full weight-bearing time and shoulder joint function were compared between the two groups.Results The operation time and hospital stay in the OBS group were shorter than those in the RP group,the amount of intraoperative blood loss and postoperative drainage volume were less than those in the RP group,and the number of intraoperative fluoroscopy was less than that in the RP group,the differences between the groups were statistically significant(P<0.05).The active time and full weight-bearing time of the OBS group were(4.93±0.65)d and(55.31±7.13)d,respectively,which were shorter than(10.37±2.65)d and(82.33±9.93)d of the RP group,and the differences between the groups were statistically significant(P<0.05).2 weeks after surgery,the Constant-Murley shoulder joint score in the OBS group was higher than that in the RP group,and the range of motion of forward flexion upward lift and abduction upward lift were higher than those in the RP group,and the differences between the groups were statistically significant(P<0.05).Conclusion Compared with RP,OBS in the treatment of USF patients can improve perioperative indexes and shoulder joint function,reduce the length of hospital stay,and is worthy of clinical application.
