电针结合集束化进食管理对脑卒中后吞咽障碍患者吞咽功能的影响

Effect of Electroacupuncture Combined with Cluster Feeding Management on Swallowing Function of Patients with Poststroke Dysphagia

目的探究脑卒中后吞咽障碍患者接受电针结合集束化进食管理对其吞咽功能的影响.方法选择本院2021年7月—2022年7月收治的80例脑卒中后吞咽障碍患者为研究对象,按随机数字表法分为对照组、观察组,每组40例.对照组采用集束化进食管理,观察组在此基础上采用电针干预,均持续4周.比较两组患者的吞咽功能、并发症发生情况及生活质量.结果干预2、4周后,观察组的洼田饮水试验评分、标准吞咽功能评估量表评分均低于对照组,改良曼恩吞咽能力评估量表评分均高于对照组,组间差异有统计学意义(P<0.05).干预2、4周后,观察组的误吸发生率分别为2.50%、7.50%,脑卒中相关性肺炎发生率分别为5.00%、7.50%,均低于对照组,组间差异有统计学意义(P<0.05).干预2、4周后,观察组的吞咽特异性生活质量量表评分分别为(125.82±8.65)分、(169.34±10.27)分,均高于对照组,组间差异有统计学意义(P<0.05).结论脑卒中后吞咽障碍患者接受电针结合集束化进食管理能有效促进吞咽功能恢复,减少并发症发生,促进生活质量改善.

Objective To investigate the effect of electroacupuncture combined with cluster feeding management on swallowing function of patients with poststroke dysphagia.Methods 80 patients with poststroke dysphagia admitted to our hospital from July 2021 to July 2022 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 40 cases in each group.The control group received cluster feeding management,and the observation group received electroacupuncture intervention on this basis,both lasted 4 weeks.The swallowing function,complications and quality of life were compared between the two groups.Results After 2 and 4 weeks of intervention,the scores of the Kubota water swallowing test and the standardized swallowing assessment in the observation group were lower than those in the control group,and the scores of the modified Mann assessment of swallowing ability was higher than that in the control group,the differences between the groups were statistically significant(P<0.05).After 2 and 4 weeks of intervention,the incidences of aspiration in the observation group were 2.50%and 7.50%,and the incidences of stroke-related pneumonia were 5.00%and 7.50%,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After 2 and 4 weeks of intervention,the swallowing-quality of life scores of the observation group were(125.82±8.65)points and(169.34±10.27)points,respectively,which were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Electroacupuncture combined with cluster feeding management can effectively promote the recovery of swallowing function,reduce complications and improve the quality of life in patients with poststroke dysphagia.
