犀角地黄汤加减用于血热型寻常型银屑病的临床价值分析

Analysis of Clinical Value of Modified Xijiao Dihuang Decoction in the Treatment of Psoriasis Vulgaris of Blood-Heat Type

目的研究血热型寻常型银屑病治疗中应用犀角地黄汤加减的效果.方法选取本院2020年1月—2021年12月收治的70例血热型寻常型银屑病患者为研究对象,采用随机数表法将其分为两组.对照组(n=35)采用常规西药治疗,研究组(n=35)在此基础上采用犀角地黄汤加减治疗.比较两组患者的中医证候积分、免疫功能指标及健康状况.结果治疗前,两组的各项中医证候积分比较,组间差异无统计学意义(P>0.05);治疗后,研究组的瘙痒、皮肤干燥、烦躁易怒、口干积分分别为(1.36±0.29)分、(1.50±0.26)分、(1.10±0.21)分、(1.21±0.25)分,均低于对照组的(1.61±0.36)分、(1.79±0.33)分、(1.38±0.26)分、(1.50±0.30)分,组间差异有统计学意义(P<0.05).治疗前,两组的各项免疫功能指标水平比较,组间差异无统计学意义(P>0.05);治疗后,研究组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平分别为(26.80±3.12)%、(35.87±3.40)%、(1.12±0.13),均低于对照组的(29.14±3.39)%、(39.09±3.61)%、(1.25±0.16),组间差异有统计学意义(P<0.05).治疗前,两组的波士顿健康调查简表(SF-36)各维度评分比较,组间差异无统计学意义(P>0.05);治疗后,研究组的SF-36各维度评分均高于对照组,组间差异有统计学意义(P<0.05).结论犀角地黄汤加减可有效缓解血热型寻常型银屑病患者的临床症状,改善其机体免疫状态及健康状况,值得临床推广使用.

Objective To study the effect of modified Xijiao Dihuang Decoction in the treatment of psoriasis vulgaris with blood-heat type.Methods 70 patients with blood-heat psoriasis vulgaris admitted to our hospital from January 2020 to December 2021 were selected as the research objects and were divided into two groups by random number table method.The control group(n=35)was treated with conventional western medicine,and the study group(n=35)was treated with modified Xijiao Dihuang Decoction on this basis.The TCM syndrome scores,immune function indexes and health status of the two groups were compared.Results Before treatment,there were no significant differences in the scores of TCM syndrome between the two groups(P>0.05);after treatment,the scores of pruritus,dry skin,irritability and dry mouth in the study group were(1.36±0.29)points,(1.50±0.26)points,(1.10±0.21)points,(1.21±0.25)points,respectively,which were lower than(1.61±0.36)points,(1.79±0.33)points,(1.38±0.26)points,(1.50±0.30)points in the control group,and the differences between the groups were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of immune function indexes between the two groups(P>0.05);after treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the study group were(26.80±3.12)%,(35.87±3.40)%,(1.12±0.13),respectively,which were lower than(29.14±3.39)%,(39.09±3.61)%,(1.25±0.16)%in the control group,the differences between the groups were statistically significant(P<0.05).Before treatment,there were no significant differences between the two groups in scores for each dimension of the MOS item short from health survey(SF-36)(P>0.05);after treatment,the scores for each dimension of SF-36 of the study group were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Modified Xijiao Dihuang Decoction can effectively relieve the clinical symptoms of patients with psoriasis vulgaris of blood-heat type,improve their immune status and health status,worthy of clinical promotion and use.
