腹腔镜辅助根治手术对结直肠癌患者免疫功能及术后并发症的影响

Effect of Laparoscopic Assisted Radical Surgery on Immune Function and Postoperative Complications in Patients with Colorectal Cancer

目的探讨腹腔镜辅助根治手术对结直肠癌(CRC)患者的治疗效果.方法选择2020年4月—2022年4月我院收治的CRC患者72例,按随机数字表法分为两组,每组36例.对照组行传统开腹手术,观察组行腹腔镜辅助根治手术.对比两组患者的术后恢复情况、免疫功能及并发症发生情况.结果观察组的术后首次排气时间、肠鸣音恢复时间、恢复正常饮食时间及住院时间分别为(52.23±4.16)h、(54.03±5.08)h、(55.46±6.19)h、(9.02±1.51)d,均短于对照组的(60.54±5.89)h、(63.89±6.12)h、(78.92±7.13)h、(10.47±1.83)d,组间差异有统计学意义(P<0.05).术后24 h,观察组的疼痛视觉模拟评分为(4.18±0.91)分,低于对照组的(5.23±1.02)分,差异有统计学意义(P<0.05).术后5 d,观察组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,组间差异有统计学意义(P<0.05).观察组的术后并发症发生率为5.56%,低于对照组的22.22%,差异有统计学意义(P<0.05).结论采用腹腔镜辅助根治手术治疗CRC,具有术后恢复快、患者痛苦小、术后并发症少等优势,且对免疫功能损伤较小,临床应用价值较高.

Objective To investigate the effect of laparoscopic assisted radical surgery on patients with colorectal cancer(CRC).Methods 72 CRC patients admitted to the hospital from April 2020 to April 2022 were selected and were divided into two groups according to random number table method,with 36 cases in each group.The control group received traditional open surgery,and the observation group received laparoscopic assisted radical surgery.The postoperative recovery,immune function and complications of the two groups were compared.Results The first postoperative exhaust time,bowel sound recovery time,normal diet recovery time and hospital stay time in the observation group were(52.23±4.16)h,(54.03±5.08)h,(55.46±6.19)h,(9.02±1.51)d,respectively,which were shorter than(60.54±5.89)h,(63.89±6.12)h,(78.92±7.13)h and(10.47±1.83)d in the control group,and the differences between the groups were statistically significant(P<0.05).24 h after surgery,the pain visual simulation scale score of the observation group was(4.18±0.91)points,which was lower than(5.23±1.02)points of the control group,and the difference was statistically significant(P<0.05).5 days after surgery,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 5.56%,lower than 22.22%in the control group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic assisted radical surgery in the treatment of CRC has the advantages of fast postoperative recovery,less pain for patients,fewer postoperative complications,and less damage to immune function,so it has high clinical application value.
