CPM锻炼仪联合量化康复训练对老年股骨粗隆间骨折患者髋关节功能的影响 被引量:1

Effect of CPM Exercise Instrument Combined with Quantitative Rehabilitation Training on Hip Function in Elderly Patients with Intertrochanteric Fracture of Femur

摘要 目的探讨持续被动(CPM)锻炼仪联合量化康复训练在老年股骨粗隆间骨折(IFF)患者中的应用效果.方法选取2020年1月—2022年1月我院收治的85例老年IFF患者,按随机数字表法分为观察组(n=43)与对照组(n=42).对照组进行量化康复训练,观察组在对照组基础上加用CPM锻炼仪干预,均持续至术后12周.对比两组关节疼痛、髋关节功能、并发症发生率及生活质量.结果治疗12周后,观察组疼痛视觉模拟评分(1.73±0.20)分低于对照组的(2.39±0.33)分,差异有统计学意义(P<0.05).治疗12周后,观察组Harris髋关节功能评分量表中的疼痛评分(30.89±2.48)分、关节活动度评分(2.68±0.17)分、关节功能评分(38.42±1.61)分、关节畸形评分(2.65±0.21)分及生活质量综合评定问卷中的心理维度评分(83.92±6.02)分、躯体维度评分(85.64±6.68)分、社会功能维度评分(82.33±5.15)分、物质生活维度评分(87.41±6.28)分均高于对照组的(26.05±3.16)分、(2.35±0.28)分、(34.52±2.82)分、(2.26±0.18)分、(75.53±7.44)分、(76.40±5.49)分、(74.27±6.06)分、(76.59±7.23)分,组间差异有统计学意义(P<0.05).观察组并发症发生率4.65%低于对照组的21.43%,差异有统计学意义(P<0.05).结论CPM锻炼仪联合量化康复训练能改善老年IFF患者的髋关节功能,减轻关节疼痛,减少并发症,提高其生活质量. Objective To explore the application effect of continuous passive motion(CPM)exercise instrument combined with quantitative rehabilitation training in elderly patients with intertrochanteric fracture of femur(IFF).Methods 85 elderly patients with IFF admitted to the hospital from January 2020 to January 2022 were selected and divided into an observation group(n=43)and a control group(n=42)according to random number table method.The control group was given quantitative rehabilitation training,and the observation group was given CPM exercise instrument on the basis of the control group,with both interventions continued until 12 weeks after operation.The joint pain,hip joint function,complication rate and quality of life of the two groups were compared.Results After 12 weeks of treatment,the pain visual analogue scale score(1.73±0.20)points of the observation group was significantly lower than(2.39±0.33)points of the control group,and the difference was statistically significant(P<0.05).After 12 weeks of treatment,the pain score(30.89±2.48)points,the range of motion score(2.68±0.17)points,the joint function score(38.42±1.61)points,the joint deformity score(2.65±0.21)points in Harris hip score,and the psychological dimension score(83.92±6.02)points,the physical dimension score(85.64±6.68)points,the social function dimension score(82.33±5.15)points,the material life dimension score(87.41±6.28)points in generic quality of life inventory-74 of the observation group were higher than(26.05±3.16)points,(2.35±0.28)points,(34.52±2.82)points,(2.26±0.18)points,(75.53±7.44)points,(76.40±5.49)points,(74.27±6.06)points and(76.59±7.23)points of the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of complications of the observation group was 4.65%,which was lower than 21.43%of the control group,and the difference was statistically significant(P<0.05).Conclusion CPM exercise instrument combined with quantitative rehabilitation training can improve hip joint function,relieve joint pain,reduce complications and improve quality of life in elderly patients with IFF.
