化瘀接骨汤结合夹板外固定对老年桡骨骨折患者骨代谢及机体炎症反应的影响

Effect of Huayu Jiegu Decoction Combined with Splint External Fixation on Bone Metabolism and Inflammatory Response in Elderly Patients with Radial Fracture

目的探讨化瘀接骨汤结合夹板外固定在老年桡骨骨折患者中的应用价值.方法选取2020年6月—2022年6月山亭区人民医院收治的74例老年桡骨骨折患者,按随机数字表法分为两组.两组均采用夹板外固定治疗,在此基础上,对照组(n=37)采用常规西医治疗,观察组(n=37)采用常规西医联合化瘀接骨汤治疗,两组均治疗12周.比较两组的疼痛程度、骨代谢指标、炎症反应指标及不良反应发生情况.结果治疗后,观察组的疼痛视觉模拟评分为(1.62±0.39)分,低于对照组的(2.35±0.47)分,差异有统计学意义(P<0.05).治疗后,观察组的骨钙素、骨保护素、骨形态发生蛋白-2水平均高于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的肿瘤坏死因子-α、C反应蛋白、白细胞介素-6、白细胞介素-1水平均低于对照组,差异有统计学意义(P<0.05).两组用药期间均未出现不良反应.结论化瘀接骨汤结合夹板外固定能减轻老年桡骨骨折患者的疼痛,抑制机体炎症反应,改善骨代谢水平,且安全可行.

Objective To investigate the application value of Huayu Jiegu Decoction combined with splint external fixation in elderly patients with radial fracture.Methods A total of 74 elderly patients with radial fracture admitted to Shanting District People's Hospital from June 2020 to June 2022 were selected and were divided into two groups according to random number table method.Both groups were treated with splint external fixation.On this basis,the control group(n=37)received conventional western medicine treatment,the observation group(n=37)received conventional western medicine combined with Huayu Jiegu Decoction.Both groups were treated for 12 weeks.The pain degree,bone metabolism indexes,inflammatory response indexes and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the pain visual analog scale score of the observation group was(1.62±0.39)points,lower than(2.35±0.47)points of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of bone gla-protein,osteoprotegerin and bone morphogenetic protein-2 in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of tumor necrosis factor-α,C-reactive protein,interleukin-6 and interleukin-1 in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).There were no adverse reactions in the two groups.Conclusion Huayu Jiegu Decoction combined with splint external fixation can relieve pain,inhibit inflammatory response and improve bone metabolism in elderly patients with radial fracture,and it is safe and feasible.
