沙库巴曲缬沙坦联合分级运动康复疗法对慢性心力衰竭患者运动耐力、心功能及心脏不良事件的影响

Effects of Shakubartrivalsartan Combined Graded Exercise Rehabilitation Therapy on Exercise Endurance,Cardiac Function and Cardiac Adverse Events in Patients with Chronic Heart Failure

目的探讨沙库巴曲缬沙坦联合分级运动康复疗法对慢性心力衰竭(CHF)患者运动耐力、心功能及心脏不良事件的影响.方法选择2019年11月—2022年1月我院收治的74例慢性心力衰竭患者,按随机数字表法将患者分为两组.对照组(n=37)采用沙库巴曲缬沙坦钠片口服治疗,观察组(n=37)在对照组基础上采取分级运动康复疗法,均持续干预3个月.比较两组运动耐力、心功能、生活质量及心脏不良事件.结果干预前,两组运动耐力、心功能、生活质量、心脏不良事件比较,组间差异无统计学意义(P>0.05);干预后,观察组6 min步行试验距离较对照组长,左室射血分数较对照组高,左室舒张末期容积、左室收缩末期容积均较对照组小,B型脑钠肽水平较对照组低,明尼苏达心衰生活质量量表总分及各维度得分均较对照组低,组间差异有统计学意义(P<0.05).观察组心脏不良事件发生率较对照组低,差异有统计学意义(P<0.05).结论沙库巴曲缬沙坦联合分级运动康复疗法在提高CHF患者运动耐力、改善心功能方面效果显著,且能减少心脏不良事件的发生风险.

Objective To investigate the effects of shakubartrivalsartan combined graded exercise rehabilitation therapy on exercise endurance,cardiac function and cardiac adverse events in patients with chronic heart failure(CHF).Methods 74 patients with chronic heart failure admitted to the hospital from November 2019 to January 2022 were selected and divided into two groups according to random number table method.The control group(n=37)was treated with shakubactril valsartan sodium tablets orally,and the observation group(n=37)was treated with graded exercise rehabilitation therapy on the basis of the control group,with continuous intervention for 3 months.Exercise endurance,cardiac function,quality of life and cardiac adverse events were compared between the two groups.Results Before intervention,there were no significant differences in exercise endurance,cardiac function,quality of life and cardiac adverse events between the two groups(P>0.05);after intervention,the 6 min walking distance of the observation group was longer than that of the control group,the left ventricular ejection fraction was higher than that of the control group,the left ventricular end-diastolic volume and the left ventricular end-systolic volume were smaller than those of the control group,the B-type brain natriuretic peptide level was lower than that of the control group,and the total score of the Minnesota heart failure Quality of Life scale and the scores of each dimension were lower than those of the control group,with statistically significant differences between the groups(P<0.05).The incidence of cardiac adverse events in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Shakubartrivalsartan combined graded exercise rehabilitation therapy has significant effects on improving exercise endurance and cardiac function in patients with CHF,and can reduce the risk of adverse cardiac events.
