摘要
目的探讨低频电刺激联合前庭康复训练对后循环缺血性眩晕患者脑血流及眩晕症状的影响.方法选择本院2020年4月—2022年4月收治的后循环缺血性眩晕患者74例,按照随机数字表法分为两组,每组37例.对照组进行前庭康复训练,观察组在此基础上采用低频电刺激治疗,两组均治疗4周.比较两组治疗前后的眩晕评定量表(DARS)、Berg平衡量表(BBS)、前庭症状指数(VSI)评分,并比较两组治疗前后的左椎动脉(LVA)、右椎动脉(RVA)、基底动脉(BA)血流速度和治疗效果.结果治疗前,两组的DARA、BBS、VSI评分比较,组间差异无统计学意义(P>0.05);治疗后,观察组的DARS、VSI评分分别为(15.21±2.18)分、(18.25±4.12)分,均低于对照组的(18.23±2.74)分、(29.34±4.85)分,BBS评分为(50.24±5.41)分,高于对照组的(38.14±4.63)分,组间差异有统计学意义(P<0.05).治疗前,两组的LVA、RVA、BA血流速度比较,组间差异无统计学意义(P>0.05);治疗后,观察组的LVA、RVA、BA血流速度分别为(36.23±6.04)cm/s、(35.14±4.93)cm/s、(37.01±7.13)cm/s,均快于对照组的(33.24±6.85)cm/s、(33.13±4.71)cm/s、(34.02±5.81)cm/s,组间差异有统计学意义(P<0.05).观察组的治疗总有效率为86.49%,高于对照组的75.68%,差异有统计学意义(P<0.05).结论低频电刺激结合前庭康复训练在后循环缺血性眩晕患者中的应用效果显著,能缓解眩晕症状,促进肢体平衡功能恢复,并能改善患者脑部血流灌注,值得临床推广使用.
Objective To investigate the effects of low frequency electrical stimulation combined with vestibular rehabilitation training on cerebral blood flow and vertigo symptoms in patients with posterior circulation ischemic vertigo.Methods A total of 74 patients with posterior circulation ischemic vertigo admitted to our hospital from April 2020 to April 2022 were selected and were divided into two groups according to random number table method,with 37 cases in each group.The control group received vestibular rehabilitation training,and the observation group received low frequency electrical stimulation on this basis.Both groups were treated for 4 weeks.The dizziness assessment rating scale(DARS),Berg balance scale(BBS)and vestibular symptom index(VSI)scores were compared between the two groups before and after treatment,and the blood flow velocity of left vertebral artery(LVA),right vertebral artery(RVA)and basilar artery(BA)before and after treatment and the therapeutic effect were compared between the two groups.Results Before treatment,there were no significant differences in DARA,BBS and VSI scores between the two groups(P>0.05);after treatment,the DARS and VSI scores of the observation group were(15.21±2.18)points and(18.25±4.12)points,respectively,lower than(18.23±2.74)points and(29.34±4.85)points of the control group,and the BBS scores was(50.24±5.41)points,which was higher than(38.14±4.63)points in the control group,and the differences between the groups were statistically significant(P<0.05).Before treatment,there were no significant differences in blood flow velocity of LVA,RVA and BA between the two groups(P>0.05);after treatment,the blood flow velocity of LVA,RVA and BA in the observation group were(36.23±6.04)cm/s,(35.14±4.93)cm/s,(37.01±7.13)cm/s,respectively,which were higher than(33.24±6.85)cm/s,(33.13±4.71)cm/s and(34.02±5.81)cm/s in the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of the observation group was 86.49%,higher than 75.68%of the control group,and the difference was statistically significant(P<0.05).Conclusion Low frequency electrical stimulation combined with vestibular rehabilitation training has a significant effect on patients with posterior circulation ischemic vertigo,which can alleviate vertigo symptoms,promote the recovery of limb balance function,improve cerebral blood perfusion.It is worthy of clinical application.
