穴位针刺联合隔药饼灸对肩周炎患者疼痛程度及肩关节活动度的影响

Effect of Acupoint Acupuncture Combined with Medicated Cake Separated Moxibustion on Pain Degree and Shoulder Joint Activity of Patients with Periarthritis of Shoulder

目的探讨穴位针刺联合隔药饼灸在肩周炎患者中的应用效果.方法选取2020年6月—2022年6月我院收治的86例肩周炎患者,按随机数字表法分为两组,各43例.对照组采取常规西药治疗,观察组在对照组基础上加用穴位针刺联合隔药饼灸治疗,均持续治疗4周.比较两组临床疗效、疼痛程度、肩关节活动度、肩关节功能、血清学指标及不良反应.结果观察组总有效率95.35%高于对照组的81.40%,差异有统计学意义(P<0.05).治疗后,观察组疼痛视觉模拟评分为(1.59±0.22)分,低于对照组的(2.08±0.29)分,肩关节前屈、后伸角度分别为(148.03±9.43)°、(35.42±3.35)°,均大于对照组的(139.83±9.22)°、(28.96±3.19)°,组间差异有统计学意义(P<0.05).治疗后,观察组Constant-Murley肩关节功能中疼痛、日常生活活动、主动活动范围、肌力评分及总分分别为(12.38±1.48)分、(17.25±2.15)分、(33.15±3.16)分、(23.34±1.16)分、(87.62±6.97)分,均高于对照组的(10.75±1.39)分、(14.29±2.07)分、(29.46±3.05)分、(21.25±1.08)分、(79.71±6.85)分,组间差异有统计学意义(P<0.05).治疗后,观察组前列腺素E_(2)(PGE_(2))、C反应蛋白(CRP)水平分别为(35.14±3.75)ng/L、(5.12±1.03)mg/L,均低于对照组的(40.36±4.11)ng/L、(6.89±1.14)mg/L,组间差异有统计学意义(P<0.05).两组不良反应发生率相比,差异无统计学意义(P>0.05).结论穴位针刺联合隔药饼灸可提高肩周炎患者临床疗效,加快肩关节功能恢复,降低血清PGE_(2)、CRP水平,且安全可靠.

Objective To investigate the effect of acupoint acupuncture combined with medicated cake moxibustion in patients with periarthritis of shoulder.Methods 86 patients with periarthritis of shoulder admitted to the hospital from June 2020 to June 2022 were selected and divided into two groups according to random number table method,with 43 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with acupoint acupuncture combined with medicine cake moxibustion for 4 weeks.The clinical efficacy,pain degree,shoulder range of motion,shoulder function,serological indexes and adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 95.35%higher than 81.40%in the control group,and the difference was statistically significant(P<0.05).After treatment,the pain visual analog score of the observation group was(1.59±0.22)points,lower than that of the control group(2.08±0.29)points,the anteriorflexion and extension angles of shoulder were(148.03±9.43)°and(35.42±3.35)°,which were higher than(139.83±9.22)°and(28.96±3.19)°in the control group,and the differences between groups were statistically significant(P<0.05).After treatment,pain,activities of daily living,active range of motion,muscle strength score and total score of Constant-Murley scale were(12.38±1.48)points,(17.25±2.15)points,(33.15±3.16)points,(23.34±1.16)points and(87.62±6.97)points,respectively,which were higher than the control group(10.75±1.39)points,(14.29±2.07)points,(29.46±3.05)points,(21.25±1.08)points,(79.71±6.85)points,the differences between groups were statistically significant(P<0.05).After treatment,the levels of prostaglandin E_(2)(PGE_(2))and C-reactive protein(CRP)in the observation group were(35.14±3.75)ng/L and(5.12±1.03)mg/L,respectively,which were lower than those in the control group(40.36±4.11)ng/L and(6.89±1.14)mg/L,and the differences were statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Acupoint acupuncture combined with medicated cake moxibustion can improve the clinical effect of patients with periarthritis of shoulder,accelerate the functional recovery of shoulder joint,reduce the serum PGE_(2) and CRP levels,and it is safe and reliable.
