中药熏蒸联合手法推拿在腰椎间盘突出症患者中的应用效果

Application Effect of Traditional Chinese Medicine Fumigation Combined with Manipulative Massage in Patients with Lumbar Disc Herniation

目的探讨腰椎间盘突出症患者采用中药熏蒸联合手法推拿干预的效果.方法选取本院2021年5月—2022年6月收治的腰椎间盘突出症患者81例,以随机数表法分为两组.对照组(n=40)进行常规康复训练,观察组(n=41)在此基础上采用中药熏蒸联合手法推拿干预.比较两组的康复效果,评估干预前后的日本骨科学会腰痛评价量表(JOA)评分、腰椎活动度及静息与活动时的疼痛视觉模拟(VAS)评分.结果观察组的康复总有效率为95.12%,高于对照组的77.50%,差异有统计学意义(P<0.05).干预后,观察组的JOA中主观症状、临床体征、日常活动受限评分分别为(5.81±0.73)分、(4.86±0.47)分、(10.95±1.42)分,均高于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的各方向腰椎活动度均大于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的静息、活动VAS评分分别为(0.92±0.43)分、(1.39±0.65)分,均低于对照组,组间差异有统计学意义(P<0.05).结论腰椎间盘突出症患者采用中药熏蒸联合手法推拿干预的效果理想,能促进腰椎功能改善及活动度提升,并能减轻疼痛,值得临床推广使用.

Objective To investigate the effect of traditional Chinese medicine fumigation combined with manipulative massage in patients with lumbar disc herniation.Methods A total of 81 patients with lumbar disc herniation admitted to the hospital from May 2021 to June 2022 were selected and were divided into two groups by random number table method.The control group(n=40)received routine nursing,and the observation group(n=41)received traditional Chinese medicine fumigation combined with manipulative massage intervention on this basis.The rehabilitation effects of the two groups were compared,and the Japanese Orthopaedic Society(JOA)low back pain evaluation scale score,lumbar range of motion,and pain visual analog scale(VAS)score of resting and moving before and after the intervention were assessed.Results The total effective rate of the observation group was 95.12%,higher than 77.50%of the control group,the difference was statistically significant(P<0.05).After intervention,the JOA scores of subjective symptoms,clinical signs and restriction of daily activities in the observation group were(5.81±0.73)points,(4.86±0.47)points and(10.95±1.42)points,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the lumbar motion in all directions of the observation group were greater than those of the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the scores of resting VAS and moving VAS in the observation group were(0.92±0.43)points and(1.39±0.65)points,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The effect of traditional Chinese medicine fumigation combined with manipulative massage in patients with lumbar disc herniation is ideal,which can promote the improvement of lumbar function and range of motion,and reduce pain,and is worthy of clinical promotion.
