独活寄生汤加减联合中医骨伤手法按摩对腰椎间盘突出症患者疼痛程度及腰椎功能的影响

Effect of Modified Duhuo Jisheng Decoction Combined with Traditional Chinese Medicine Bone Injury Manipulation Massage on Pain Degree and Lumbar Function of Patients with Lumbar Disc Herniation

目的探究独活寄生汤加减联合中医骨伤手法按摩对腰椎间盘突出症患者疼痛程度及腰椎功能的影响.方法选取济宁市中西医结合医院及济宁医学院附属医院2020年3月—2022年6月收治的90例腰椎间盘突出症患者为研究对象,按照随机数字表法分成两组.对照组(n=45)采用中医骨伤手法按摩治疗,研究组(n=45)在对照组基础上采用独活寄生汤加减治疗,均持续治疗4周.对比两组治疗前后的疼痛程度、腰椎功能,并评价治疗安全性.结果治疗后,两组的疼痛评级指数(PRI)、视觉模拟(VAS)及现时疼痛强度(PPI)、Oswestry功能障碍指数(ODI)评分均低于治疗前,日本骨科协会评估治疗分数量表(JOA)评分均高于治疗前,且研究组PRI、VAS、PPI、ODI评分均低于对照组,JOA评分高于对照组,组间差异有统计学意义(P<0.05);研究组的不良反应总发生率8.89%与对照组的6.67%相近,差异无统计学意义(P>0.05).结论为腰椎间盘突出症患者提供独活寄生汤加减、中医骨伤手法按摩联合治疗能得到确切效果,能够减轻机体疼痛症状,使腰椎功能恢复,且安全性高,值得临床推广.

Objective To explore the effect of Duhuo Jisheng Decoction combined with traditional Chinese medicine massage on the pain degree and lumbar function of patients with lumbar disc herniation.Methods 90 patients with lumbar disc herniation admitted to Jining Integrated Traditional Chinese and Western Medicine Hospital and the Affiliated Hospital of Jining Medical College from March 2020 to June 2022 were selected as the research objects,and were divided into two groups according to the random number table method.The control group(n=45)was treated with traditional Chinese medicine bone injury manipulation massage treatment,and the study group(n=45)was treated with modified Duhuo Jisheng Decoction treatment on the basis of the control group,all of which lasted for 4 weeks.The pain degree and lumbar function of the two groups before and after treatment were compared,and the treatment safety was evaluated.Results After treatment,the pain rating index(PRI),pain visual analogue scale(VAS),present pain intensity(PPI)and Oswestry disability index(ODI)scores of the two groups were lower than those before treatment,and the PRI,VAS,PPI and ODI scores of the study group were lower than those of the control group,JOA score was higher than that of the control group,with statistically significant differences between the groups(P<0.05);the total incidence of adverse reactions in the study group was 8.89%,similar to 6.67%in the control group,with no statistically significant difference(P>0.05).Conclusion The combined treatment of modified Duhuo Jisheng Decoction and traditional Chinese medicine bone injury manipulation massage for patients with lumbar disc herniation can achieve the exact effect,reduce the body pain symptoms,and restore the lumbar function,with high safety,which is worthy of clinical promotion.
