髓内钉微创手术后持续性关节被动活动对胫腓骨骨折患者疼痛程度、下肢运动功能、日常生活能力、生活质量的影响

Effects of Continuous Passive Joint Movement on Pain Degree,Lower Limb Motor Function,Daily Living Ability and Quality of Life in Patients with Tibiofibular Fracture after Minimally Invasive Intramedullary Nail Surgery

摘要 目的分析髓内钉微创手术后持续性关节被动活动对胫腓骨骨折患者疼痛程度、下肢运动功能、日常生活能力、生活质量的影响.方法选取2015年1月—2022年1月聊城市茌平区第三人民医院收治的80例胫腓骨骨折患者,按随机数字表法分为两组,各40例.两组均行髓内钉微创手术治疗,对照组术后行常规康复训练,观察组术后行持续性关节被动活动训练,均连续治疗3个月.对比两组疼痛程度、下肢运动功能、日常生活活动能力、生活质量、并发症.结果治疗前,两组疼痛视觉模拟(VAS)评分、下肢Fugl-Meyer运动功能评分量表(FMA)评分、日常生活能力量表(ADL)评分以及生活质量综合评定问卷(GQOLI-74)评分相比,组间差异无统计学意义(P>0.05);治疗3个月后,观察组VAS评分低于对照组,下肢FMA、ADL与GQOLI-74评分均高于对照组,组间差异有统计学意义(P<0.05).两组并发症发生率相当,差异无统计学意义(P>0.05).结论髓内钉微创手术后持续性关节被动活动在胫腓骨骨折患者中作用明显,能够减轻患者疼痛,促进下肢功能改善,以此有效提升其日常生活能力,改善生活质量,且不增加并发症的发生. Objective To analyze the effect of continuous passive joint movement on pain degree,lower limb motor function,daily living ability and quality of life in patients with tibiofibular fracture after minimally invasive intramedullary nail surgery.Methods 80 patients with tibiofibular fracture admitted to the Third People's Hospital of Chiping District,Liaocheng City,from January 2015 to January 2022 were selected and divided into two groups according to the random number table method,with 40 patients in each group.Both groups underwent minimally invasive intramedullary nail surgery,the control group received routine rehabilitation training after surgery,and the observation group received continuous passive joint activity training,with both interventions for 3 months.The pain degree,lower limb motor function,daily living ability,quality of life and complications of the two groups were compared.Results Before treatment,there were no statistically significant differences between the two groups in the scores of pain visual analogue scale(VAS),lower limb Fugl-Meyer assessment(FMA),activity of daily living scale(ADL)and generic quality of life inventory-74(GQOLI-74)(P>0.05);after 3 months of treatment,the VAS score of the observation group was lower than that of the control group,and the scores of lower limb FMA,ADL and GQOLI-74 were higher than those of the control group,with statistically significant differences between the groups(P<0.05).The complications rates of the two groups were similar,with no statistically significant difference(P>0.05).Conclusion Continuous passive joint movement after minimally invasive intramedullary nail surgery plays an important role in patients with tibiofibular fractures,which can reduce the pain of patients,promote the improvement of lower limb function,so as to effectively enhance their daily life ability and improve the quality of life,without increasing the occurrence of complications.
