体外超声波碎石术对输尿管结石患者碎石效果及生活质量的影响

Effect of Extracorporeal Ultrasonic Lithotripsy on Lithotripsy Effect and Quality of Life in Patients with Ureteral Calculi

目的探讨针对输尿管结石(UC)患者实施体外超声波碎石术的临床效果.方法选择2020年6月—2022年5月我院收治的78例UC患者为研究对象,按随机数字表法分为两组,各39例.对照组采用钬激光碎石术,观察组采用体外超声波碎石术,均持续随访3个月.对比两组碎石效果、围术期指标、并发症以及生活质量.结果观察组临床总有效率94.87%高于对照组的79.49%,差异有统计学意义(P<0.05);观察组结石排出、血尿持续时间分别为(3.32±0.42)d、(2.11±0.30)d,均短于对照组的(5.44±0.72)d、(3.46±0.42)d,组间差异有统计学意义(P<0.05);观察组并发症发生率5.13%低于对照组的20.51%,差异有统计学意义(P<0.05);观察组治疗后3个月生活质量综合评定问卷中的心理功能、躯体功能、社会功能及物质生活状态评分分别为(82.83±6.33)分、(81.86±7.22)分、(82.85±7.26)分、(82.86±6.95)分,均高于对照组的(72.79±5.69)分、(73.17±6.48)分、(73.74±6.77)分、(71.76±6.44)分,组间差异有统计学意义(P<0.05).结论针对UC患者实施体外超声波碎石术,疗效显著,能够缩短结石排出时间及血尿持续时间,减少并发症的发生,提升生活质量.

Objective To investigate the clinical effect of extracorporeal ultrasonic lithotripsy for patients with ureteral calculi(UC).Methods 78 patients with UC admitted to the hospital from June 2020 to May 2022 were selected as the research subjects and divided into two groups according to random number table method,39 patients in each group.The control group received holmium laser lithotripsy,and the observation group received extracorporeal ultrasonic lithotripsy.Both groups were followed up continuously for 3 months.The lithotripsy effect,perioperative indexes,complications and quality of life were compared between the two groups.Results The total clinical effective rate of the observation group was 94.87%,which was higher than 79.49%of the control group,the difference was statistically significant(P<0.05);the time of calculi discharge and duration of hematuria in the observation group were(3.32±0.42)d and(2.11±0.30)d,respectively,which were shorter than(5.44±0.72)d and(3.4±0.42)d in the control group,and the differences between the groups were statistically significant(P<0.05);the incidence of complications in the observation group was 5.13%,which was lower than 20.51%in the control group,and the difference was statistically significant(P<0.05);3 months after treatment,the scores of psychological function,physical function,social function and material life status in the generic quality of Life inventory-74 of the observation group were(82.83±6.33)points,(81.86±7.22)points,(82.85±7.26)points and(82.86±6.95)points,respectively,which were higher than(72.79±5.69)points,(73.17±6.48)points,(73.74±6.77)points and(71.76±6.44)points in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Extracorporeal ultrasonic lithotripsy for UC patients has a significant effect,which can shorten the time of calculi discharge and the duration of hematuria,reduce the occurrence of complications,and improve the quality of life.
