超选择子宫动脉栓塞术在子宫腺肌病患者中的应用效果

Application Effect of Superselective Uterine Artery Embolization in Patients with Adenomyosis

目的探究超选择子宫动脉栓塞术(SUAE)与腹腔镜手术在子宫腺肌病(AM)患者中的应用效果.方法选取2020年8月—2022年1月我院收治的70例AM患者,采用随机数字表法分为两组,每组35例.对照组采用腹腔镜下病灶切除术治疗,观察组采用SUAE治疗.比较两组的手术指征、子宫体积、糖类抗原125(CA125)水平、卵巢功能及不良反应发生情况.结果观察组的手术时间及术后住院时间均短于对照组,术中出血量少于对照组,组间差异有统计学意义(P<0.05).术后,观察组的子宫体积小于对照组,CA125水平低于对照组,组间差异有统计学意义(P<0.05).术后6个月,观察组的血清卵泡刺激素、黄体生成素、雌二醇水平分别为(8.98±0.47)IU/L、(9.80±1.15)IU/L、(9.23±1.18)pmol/L,均高于对照组,组间差异有统计学意义(P<0.05).两组的并发症发生率比较,差异无统计学意义(P>0.05).结论采用SUAE治疗子宫腺肌病可减少术中出血量,缩短手术时间及术后住院时间,降低CA125水平,减小子宫体积,改善患者卵巢功能,是一种安全、有效的治疗方式.

Objective To explore the application effect of superselective uterine artery embolization(SUAE)and laparoscopic surgery in patients with adenomyosis(AM).Methods 70 AM patients admitted to our hospital from August 2020 to January 2022 were selected and were divided into two groups by random number table method,with 35 cases in each group.The control group was treated with laparoscopic focus resection,and the observation group was treated with SUAE.The surgical indications,uterine volume,carbohydrate antigen 125(CA125)level,ovarian function and incidence of adverse reactions were compared between the two groups.Results The operation time and postoperative hospital stay of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,the differences between the groups were statistically significant(P<0.05).After surgery,the uterine volume and CA125 level of the observation group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).6 months after surgery,the serum levels of follicle stimulating hormone,luteinizing hormone and estradiol in the observation group were(8.98±0.47)IU/L,(9.80±1.15)IU/L and(9.23±1.18)pmol/L,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion SUAE is a safe and effective treatment for adenomyosis,which can reduce intraoperative blood loss,shorten operation time and postoperative hospital stay,reduce CA125 level,reduce uterine volume and improve ovarian function.
