重复经颅磁刺激联合音乐疗法对孤独症患儿认知功能及临床症状的影响

Effects of Repetitive Transcranial Magnetic Stimulation Combined with Music Therapy on Cognitive Function and Clinical Symptoms in Children with Autism

目的探讨重复经颅磁刺激(rTMS)联合音乐疗法对孤独症患儿认知功能及临床症状的影响.方法选取2019年1月—2022年5月我院收治的62例孤独症患儿为研究对象,根据随机数表法分为两组.对照组(n=31)采用常规康复训练,观察组(n=31)在对照组基础上联用rTMS联合音乐疗法,均干预3个月.比较两组认知功能、临床症状改善情况、希望水平.结果干预前,两组认知功能自评量表评分、孤独症儿童行为评定量表评分、Herth希望指数量表(HHI)评分比较,组间差异无统计学意义(P>0.05);干预后,观察组语言流畅性评分(16.63±1.52)分、视觉持续注意力评分(17.21±1.52)分、短时口语记忆评分(17.25±2.36)分、HHI评分(43.71±2.35)分,均高于对照组的(12.26±1.48)分、(13.54±1.23)分、(12.07±2.18)分、(41.59±2.13)分,感觉评分(8.54±1.05)分、交往评分(9.47±1.04)分、躯体和物体使用评分(9.44±1.01)分、语言评分(9.63±1.29)分、社会生活自理评分(8.53±1.59)分,均低于对照组的(10.15±1.11)分、(11.48±1.15)分、(11.67±1.15)分、(12.56±1.51)分、(11.26±1.12)分,组间差异有统计学意义(P<0.05).结论rTMS联合音乐疗法应用于孤独症患儿中,可改善其认知功能及临床症状,提升其希望水平.

Objective To investigate the effects of repetitive transcranial magnetic stimulation(rTMS)combined with music therapy on cognitive function and clinical symptoms in children with autism.Methods 62 autistic children admitted to the hospital from January 2019 to May 2022 were selected as the research objects,and were divided into two groups according to the random number table.The control group(n=31)received routine rehabilitation training,while the observation group(n=31)received rTMS combined with music therapy on the basis of the control group,and both groups were intervened for 3 months.The cognitive function,clinical symptom improvement and hope level of the two groups were compared.Results Before intervention,there were no significant differences in the scores of cognitive function self-rating scale,autism behavior checklist and Herth hope index(HHI)between the two groups(P>0.05);After the intervention,the verbal fluency score,visual sustained attention score,short-term oral memory score and HHI score of the observation group were(16.63±1.52)points,(17.21±1.52)points,(17.25±2.36)points and(43.71±2.35)points,respectively,which were higher than(12.26±1.48)points,(13.54±1.23)points,(12.07±2.18)points and(41.59±2.13)points of the control group,sensory score,communication score,body and object use score,language score and self-care score in social life were(8.54±1.05)points,(9.47±1.04)points,(9.44±1.01)points,(9.63±1.29)points and(8.53±1.59)points,respectively,which were lower than(10.15±1.11)points,(11.48±1.15)points,(11.67±1.15)points,(12.56±1.51)points and(11.26±1.12)points of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The combination of rTMS and music therapy in autistic children can improve their cognitive function and clinical symptoms,and enhance their hope level.
