低切迹微型接骨板加可折断式带孔克氏针钢缆髓内外固定治疗MayoⅡB、Ⅲ型尺骨鹰嘴骨折临床疗效观察

Clinical Observation of Low Notch Micro Bone Plate and Broken Steel Cable with Kirschner Wire Internal and External Fixation in the Treatment of MayoⅡB andⅢType Olecranon Fracture

目的研究探讨低切迹微型接骨板加可折断式带孔克氏针钢缆髓内外固定治疗MayoⅡB、Ⅲ型尺骨鹰嘴骨折的临床疗效.方法以2017年1月—2022年2月我院骨伤科收治的尺骨鹰嘴骨折MayoⅡB、Ⅲ型患者21例为研究对象,均行尺骨鹰嘴骨折切开复位低切迹微型接骨板加可折断式带孔克氏针钢缆髓内外固定术,手术后定期门诊随访.观察患者尺骨鹰嘴骨折的愈合状况,评定其肘关节疼痛、运动、稳定性及日常生活功能.结果21例患者术后随访3~6个月,均达到骨折一期愈合,无接骨板、螺钉及克氏针松动、脱出.术后3个月以Mayo系统进行评分,优良率为95.2%.结论MayoⅡB、Ⅲ型尺骨鹰嘴骨折患者采用低切迹微型接骨板加可折断式带孔克氏针钢缆髓内外固定治疗,能有效避免创口延迟愈合、感染等,减少内固定松动移位,有助于患者早期活动肘关节,具有确切的临床效果.

Objective To investigate the clinical effect of low notch micro bone plate and broken steel cable with Kirschner wire internal and external fixation in the treatment of MayoⅡB andⅢulna olecranon fractures.Methods From January 2017 to February 2022,21 patients with ulna olecranon fractures of MayoⅡB andⅢadmitted to Department of Orthopedics of our hospital were selected as the research objects.All patients underwent open reduction of olecranon fracture with low notch micro bone plate and broken steel cable with Kirschner wire internal and external fixation.Regular outpatient follow-up was performed after operation.The healing status of ulna olecranon fracture was observed,and elbow pain,motion,stability and daily living function were evaluated.The excellent and good rate was 95.2%evaluated by Mayo system 3 months after operation.Results 21 patients were followed up for 3-6 months after operation,and all of them achieved primary fracture healing without loosening or prolapse of bone plate,screw and Kirschner wire.3 months after the operation,the Mayo system was used to score the excellent rate of 95.2%.Conclusion The use of low notch micro bone plate and broken steel cable with Kirschner wire internal and external fixation in the treatment of ulna olecranon fractures of MayoⅡB andⅢcan effectively avoid delayed nonunion and infection of wounds,reduce loosening and displacement of internal fixation,and help patients to move the elbow joint early,with exact clinical effect.
