布地奈德联合螺内酯及双克治疗早产儿支气管肺发育不良临床效果及对炎症反应的影响

Clinical Effect of Budesonide Combined with Spironolactone and Digram in the Treatment of Bronchopulmonary Dysplasia in Premature Infants and Its Effect on Inflammation

摘要 目的探讨布地奈德联合螺内酯及双克治疗在早产儿支气管肺发育不良(BPD)中的应用价值.方法选择2020年7月—2022年7月该院收治的74例BPD早产患儿,按随机数字表法分为两组,各37例.对照组采用螺内酯及双克治疗,观察组采用布地奈德联合螺内酯及双克治疗,均治疗14 d.对比分析两组临床疗效、炎症反应、血气指标、不良反应.结果观察组治疗总有效率为94.59%,高于对照组的78.38%,差异有统计学意义(P<0.05).治疗后,观察组白细胞介素-6水平(0.42±0.03)ng/L、C反应蛋白水平(0.42±0.08)mg/L、降钙素原水平(0.30±0.09)ng/mL、中性粒细胞百分比(35.18±2.29)%、动脉血二氧化碳分压(45.29±3.67)mmHg均低于对照组,动脉血氧分压(56.03±5.89)mmHg高于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论布地奈德联合螺内酯及双克治疗早产儿BPD,能够有效减轻其机体炎症反应,改善血气指标,且安全可靠,临床应用价值更高. Objective To investigate the application value of budesonide combined with spironolactone and bigram in the treatment of premature infant with bronchopulmonary dysplasia(BPD).Methods A total of 74 premature infant with BPD admitted to the hospital from July 2020 to July 2022 were divided into two groups according to random number table method,with 37 cases in each group.The control group was treated with spironolactone and bigram,and the observation group was treated with budesonide combined with spironolactone and bigram for 14 days.The clinical efficacy,inflammatory response,blood gas index and adverse reactions of the two groups were compared and analyzed.Results The total effective rate of the observation group was 94.59%,which was higher than 78.38%of the control group,and the difference was statistically significant(P<0.05).After treatment,interleukin-6 level(0.42±0.03)ng/L,C-reactive protein level(0.42±0.08)mg/L,procalcitonin level(0.30±0.09)ng/mL,neutrophil percentage(35.18±2.29)%,partial pressure of carbon dioxide in artery(45.29±3.67)mmHg in the observation group were lower than those of the control group,and the partial oxygen pressure of arterial blood was(56.03±5.89)mmHg,which was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Budesonide combined with spironolactone and bigram in the treatment of premature infant BPD can effectively reduce the body inflammatory response and improve blood gas indexes,and is safe and reliable,with higher clinical application value.
