腹腔镜胆囊切除术治疗结石性胆囊炎的临床效果

Clinical Effect of Laparoscopic Cholecystectomy in the Treatment of Calculous Cholecystitis

目的探讨腹腔镜胆囊切除术(LC)治疗结石性胆囊炎的临床效果.方法选取我院2020年3月—2022年6月收治的73例结石性胆囊炎患者,随机分为对照组36例与研究组37例.对照组采取小切口胆囊切除术治疗,研究组采取LC治疗.比较两组术后疼痛程度、围术期情况、炎症因子水平及并发症发生情况.结果术后第1、4天,研究组疼痛视觉模拟评分均低于对照组,组间差异有统计学意义(P<0.05).研究组首次下床活动时间、肠鸣音恢复时间、住院时间均短于对照组,组间差异有统计学意义(P<0.05).术后第1、4天,研究组的白细胞介素-6、肿瘤坏死因子-α、C反应蛋白水平均低于对照组,组间差异有统计学意义(P<0.05).两组并发症发生率比较,差异无统计学意义(P>0.05).结论LC治疗结石性胆囊炎的临床疗效显著,患者术后疼痛及炎性应激反应轻,且不会增加并发症的发生风险,安全性高.

Objective To investigate the clinical effect of laparoscopic cholecystectomy(LC)in the treatment of calculous cholecystitis.Methods A total of 73 patients with calculi cholecystitis admitted to the hospital from March 2020 to June 2022 were selected and randomly divided into the control group of 36 cases and the study group of 37 cases.The control group was treated with small-incision cholecystectomy,while the study group was treated with LC.Postoperative pain,perioperative conditions,inflammatory factor levels and complications were compared between the two groups.Results On the 1st and 4th day after surgery,pain visual anoalogue scale scores of the study group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).The first time of getting out of bed,the recovery time of bowel sound and the length of hospital stay in the study group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).On the 1st and 4th day after surgery,the levels of interleukin-6,tumor necrosis factor-αand C-reactive protein in the study group were lower than those in the control group,with statistical significance differences between the groups(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The clinical efficacy of LC in the treatment of calculous cholecystitis is significant,the postoperative pain and inflammatory stress reaction of patients is light,and does not increase the risk of complications,with high safety.
