综合消肿疗法联合渐进式阻力训练对乳腺癌相关淋巴水肿患者患臂周径及淋巴水肿程度的影响

Effects of Comprehensive Detumescence Therapy Combined with Progressive Resistance Training on Arm Circumference and Degree of Lymphedema in Patients with Breast Cancer Related Lymphedema

目的探讨综合消肿疗法联合渐进式阻力训练在乳腺癌相关淋巴水肿(BCRL)患者中的应用效果.方法选取2020年5月—2022年4月本院收治的94例BCRL患者,按随机数字表法分为两组,每组47例.对照组采用综合消肿疗法,观察组在此基础上进行渐进式阻力训练,均持续干预8周.对比两组患者的患臂周径、淋巴水肿程度、患肢活动度及生存质量.结果干预后,观察组的患臂周径为(17.10±0.52)cm,小于对照组的(18.38±0.94)cm,淋巴水肿程度轻于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的肩关节内旋、外展、后伸、前屈角度均大于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的生活质量测定量表中情感状况、生理状况、功能状况、社会/家庭状况、附加关注评分均高于对照组,组间差异有统计学意义(P<0.05).结论综合消肿疗法联合渐进式阻力训练能减轻BCRL患者的淋巴水肿程度,增加患肢活动度,提高术后生存质量.

Objective To investigate the application effect of comprehensive detumescence therapy combined with progressive resistance training in patients with breast cancer related lymphedema(BCRL).Methods A total of 94 BCRL patients admitted to our hospital from May 2020 to April 2022 were selected and were divided into two groups according to random number table method,with 47 cases in each group.The control group received comprehensive detumescence therapy,while the observation group received progressive resistance training on this basis.Both groups continued to intervene for 8 weeks.The circumference diameter of the affected arm,the degree of lymphedema,the range of motion of the affected limb and the quality of life were compared between the two groups.Results After intervention,the circumference of the affected arm in the observation group was(17.10±0.52)cm,which was smaller than(18.38±0.94)cm in the control group,and the degree of lymphedema was less than that in the control group,the differences between the groups were statistically significant(P<0.05).After intervention,the angles of internal rotation,abduction,extension and flexion of the shoulder in the observation group were greater than those in the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the scores of emotional status,physiological status,functional status,social/family status and additional attention in the function assessment of cancer therapy-breast of the observation group were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Comprehensive detumescence therapy combined with progressive resistance training can reduce the degree of lymphedema in BCRL patients,increase the range of motion of affected limbs,and improve the postoperative quality of life.
