关节镜下清理术联合囊肿口扩大引流术治疗腘窝囊肿的临床效果观察

Clinical Observation of Arthroscopic Debridement Combined with Expanded Drainage of Cyst Orifice in the Treatment of Popliteal Cyst

目的观察腘窝囊肿患者行关节镜下清理术联合囊肿口扩大引流术治疗的效果.方法选取我院2019年7月—2021年7月收治的44例腘窝囊肿患者为研究对象,按照奇偶数分组法分为对照组和试验组,每组22例.对照组采用开放手术治疗,试验组采用关节镜下清理术联合囊肿口扩大引流术治疗.比较两组手术相关指标、Lysholm膝关节评分(LKS)、疼痛视觉模拟评分(VAS)及疾病复发率.结果试验组手术时间(59.85±4.38)min长于对照组的(48.97±5.71)min,切口长度(1.18±0.49)cm短于对照组的(9.60±3.22)cm、术中出血量(11.24±5.63)mL少于对照组的(52.35±6.72)mL、术后疼痛持续时间(1.11±0.78)d、住院时间(6.12±1.20)d均短于对照组的(4.15±1.57)d、(11.36±2.18)d,组间差异有统计学意义(P<0.05).试验组术后6个月的LKS评分(89.87±7.16)分高于对照组的(73.42±5.32)分,术后3 d的VAS评分(0.81±0.46)分低于对照组的(2.43±0.82)分,组间差异有统计学意义(P<0.05).试验组术后12个月的疾病复发率9.09%低于对照组的40.91%,差异有统计学意义(P<0.05).结论关节镜下清理术联合囊肿口扩大引流术治疗腘窝囊肿的效果较好,手术相关指标较为理想,患者膝关节功能改善明显,术后疼痛轻.

Objective To observe the effect of arthroscopic debridement combined with expanded drainage of cyst orifice in the treatment of popliteal cyst.Methods 44 patients with popliteal cyst admitted to the hospital from July 2019 to July 2021 were selected as the research object,and were divided into a control group and a test group according to odd even number grouping method,with 22 patients in each group.The control group was treated with open surgery,while the test group was treated with arthroscopic debridement combined with expanded drainage of cyst orifice.The operation related indexes,Lysholm knee score(LKS),pain visual analog score(VAS)and disease recurrence rate were compared between the two groups.Results The operation time of the test group was(59.85±4.38)min,which was longer than(48.97±5.71)min of the control group,the incision length was(1.18±0.49)cm,which was shorter than(9.60±3.22)cm of the control group,the intraoperative bleeding volume was(11.24±5.63)mL,which was less than(52.35±6.72)mL of the control group,the postoperative pain duration was(1.11±0.78)d,and the hospital stay was(6.12±1.20)d,which were shorter than(4.15±1.57)d,(11.36±2.18)d of the control group,and the differences between the groups were statistically significant(P<0.05).The LKS score(89.87±7.16)points of the test group was higher than(73.42±5.32)points of the control group at 6 months after operation,and the VAS score(0.81±0.46)points at 3 days after operation was lower than(2.43±0.82)points of the control group,with significant differences between the groups(P<0.05).The disease recurrence rate of the test group was 9.09%12 months after surgery,which was lower than 40.91%of the control group(P<0.05).Conclusion Arthroscopic debridement combined with expanded drainage of cyst orifice for popliteal cyst has a good effect,and the operation related indexes are ideal.The knee joint function of the patient has been improved significantly,and the postoperative pain is slight.
