摘要
目的探究急性脑梗死后运动性失语患者采用经颅磁刺激(TMS)联合语言康复对其神经功能及语言功能的影响.方法选择该院2020年12月—2022年5月收治的84例急性脑梗死后运动性失语患者为研究对象,按随机数字表法分为对照组和观察组,各42例.对照组采用语言康复治疗,观察组在对照组基础上联合TMS治疗,均持续治疗4周.比较两组神经功能、语言功能、临床疗效.结果两组治疗前神经、语言功能对比,组间差异无统计学意义(P>0.05);观察组治疗后美国国立卫生研究院卒中量表评分(9.15±0.84)分低于对照组,西方失语症成套测验中自发言语评分(13.30±2.11)分、听觉理解评分(138.49±15.63)分、复述检查评分(78.28±4.89)分、命名检查评分(73.42±3.39)分及AQ评分(76.59±5.80)分均高于对照组,组间差异有统计学意义(P<0.05).观察组治疗总有效率95.24%高于对照组的78.57%,差异有统计学意义(P<0.05).结论TMS联合语言康复治疗能够有效减轻急性脑梗死后运动性失语患者的神经功能损害,改善语言功能,可于临床推广.
Objective To investigate the effects of transcranial magnetic stimulation(TMS)combined with language rehabilitation on the neurological and language functions in patients with motor aphasia after acute cerebral infarction.Methods 84 patients with motor aphasia after acute cerebral infarction admitted to the hospital from December 2020 to May 2022 were selected as the research objects.They were randomly divided into a control group and an observation group,with 42 patients in each group.The control group was treated with language rehabilitation,and the observation group was treated with TMS on the basis of control group,and both groups were treated for 4 weeks.The neurological function,language function and clinical efficacy of the two groups were compared.Results There was no significant difference in neurological and language function between the two groups before treatment(P>0.05);After treatment,the National Institute of Health stroke scale score(9.15±0.84)points in the observation group was lower than that in the control group,the spontaneous speech score(13.30±2.11)points,auditory comprehension score(138.49±15.63)points,retelling inspection score(78.28±4.89)points,nominative test score(73.42±3.39)points and AQ score(76.59±5.80)points of the western aphasia battery in the observation group were all higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of the observation group was 95.24%,which was higher than 78.57%of the control group,and the difference was statistically significant(P<0.05).Conclusion TMS combined with language rehabilitation therapy can effectively reduce neurological function impairment and improve speech function in patients with motor aphasia after acute cerebral infarction,which can be popularized in clinic.
