不同任务模式的强制性运动疗法对脑卒中患者平衡及下肢功能的影响

Effects of Constraint-Induced Movement Therapy with Different Task Modes on Balance and Lower Limb Function in Stroke Patients

摘要 目的探究两种不同任务模式的强制性运动疗法(CIMT)对脑卒中患者平衡功能及下肢运动功能的影响.方法选取该院康复医学科2019年9月—2021年12月收治的75例脑卒中患者为研究对象,按随机数字表法分为三组,每组25例.对照组采用常规康复治疗,rCIMT组采取每天400次重复任务训练,mCIMT组采取每天3 h任务训练,均持续治疗6周.比较两组Berg平衡量表(BBS)评分、稳定性指数(SI)评分、Fugl-Meyer运动功能评定量表下肢部分(FMA-LE)评分和10 m最大步行速度(10 mMWS).结果治疗后,三组患者的BBS评分、FMA-LE评分、SI评分及10 mMWS均较治疗前改善,且rCIMT组和mCIMT组的BBS评分、FMA-LE评分均高于对照组,SI评分低于对照组,10 mMWS快于对照组,组间差异有统计学意义(P<0.05),但rCIMT组和mCIMT组的BBS评分、FMA-LE评分、SI评分及10 mMWS比较,组间差异无统计学意义(P>0.05).结论CIMT可明显改善脑卒中患者的平衡能力及下肢运动功能,以重复次数为训练强度衡量标准的CIMT方案可以作为mCIMT的替代方案. Objective To explore the effects of two different task modes of constraint-induced movement therapy(CIMT)on balance function and lower limb motor function in stroke patients.Methods A total of 75 stroke patients admitted to the Department of Rehabilitation Medicine of the hospital from September 2019 to December 2021 were selected as the research objects and divided into three groups according to the random number table method,with 25 cases in each group.The control group received routine rehabilitation treatment,the rCIMT group received 400 times of repetitive task training every day,and the mCIMT group received 3 hours of task training every day.The treatment lasted for 6 weeks.The Berg balance scale(BBS)score,stability index(SI)score,Fugl-Meyer assessment-lower extremity(FMA-LE)score and 10 m maximum walking speed(10 mMWS)were compared between the two groups.Results After treatment,the BBS score,FMA-LE score,SI score and 10 mMWS of the three groups were improved compared with those before treatment,and the BBS score and FMA-LE score of the rCIMT and mCIMT groups were higher than those of the control group,and the SI score was lower than that of the control group,and the 10 mMWS was faster than that of the control group,with significant differences between the two groups(P<0.05),but there were no significant difference in BBS score,FMA-LE score,SI score and 10 mMWS between rCIMT group and mCIMT group(P>0.05).Conclusion CIMT can significantly improve the balance ability and lower limb motor function of stroke patients.CIMT regimen based on the number of repetitions as the measurement standard of training intensity can be used as an alternative to mCIMT.
