依托考昔联合非布司他对痛风伴轻中度肾功能不全患者炎症反应及疼痛程度的影响

Effect of Etoricoxib Combined with Febuxostat on Inflammatory Response and Pain Level in Patients with Gout with Mild to Moderate Renal Insufficiency

目的探讨依托考昔联合非布司他在痛风伴轻中度肾功能不全患者治疗中的效果.方法选择2018年7月—2022年1月我院收治的60例痛风伴轻中度肾功能不全患者,按随机数字表法分为两组,各30例.对照组采用非布司他治疗,观察组在对照组基础上加用依托考昔治疗,两组均持续治疗2个月.对比两组临床疗效、炎症反应、疼痛程度及不良反应发生率.结果观察组临床总有效率93.33%高于对照组的73.33%,差异有统计学意义(P<0.05).治疗后,观察组糖类抗原724、C反应蛋白、白细胞介素-1β水平分别为(8.46±2.07)U/mL、(7.46±1.66)mg/L、(36.32±5.60)ng/L,均低于对照组,组间差异有统计学意义(P<0.05).观察组治疗后的尿酸水平(302.07±61.69)μmol/L、疼痛视觉模拟评分(1.38±0.25)分,均低于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论痛风伴轻中度肾功能不全患者采用依托考昔联合非布司治疗的效果显著,可有效减其轻炎症反应与疼痛程度,改善肾功能,且安全性高.

Objective To investigate the effect of etoricoxib combined with febuxostat in the treatment of patients with gout with mild to moderate renal insufficiency.Methods 60 patients with gout with mild to moderate renal insufficiency admitted to our hospital from July 2018 to January 2022 were selected and divided into two groups of 30 cases each according to the random number table method.The control group was treated with febuxostat,and the observation group was treated with etoricoxib on top of the control group,and both groups were treated continuously for 2 months.The clinical efficacy,inflammatory response,pain level and incidence of adverse reactions were compared between the two groups.Results The total clinical efficiency of the observation group was 93.33%higher than 73.33%in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of glycoantigen 724,C-reactive protein and interleukin-1βin the observation group were(8.46±2.07)U/mL,(7.46±1.66)mg/L and(36.32±5.60)ng/L,respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The uric acid level(302.07±61.69)μmol/L and pain visual analog score(1.38±0.25)points in the observation group after treatment were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).When comparing the incidence of adverse reactions between the two groups,the difference was not statistically significant(P>0.05).Conclusion Patients with gout with mild to moderate renal insufficiency are treated with etoricoxib in combination with febuxostat,which can effectively reduce their mild inflammatory response and pain level,improve renal function,and have a high safety profile.
